An interesting and exciting permanent position for band 6 RGN to join the Continuing Healthcare team in the Central Area covering the localities of Conwy and Denbighshire.
Fixed term position until the end of March 2027 to join the Continuing Health Care team in Central area, Conwy and Denbighshire. The post holder will manage on behalf of the Central division the implementation, monitoring and reviewing of NHS Funded Registered Nursing Care (FNC) and Continuing NHS Health Care (CHC) in Care Homes (Nursing) and in the domiciliary setting to ensure patients receive care of a quality which meets best practice and national standards as prescribed and funded by the NHS.
Main duties of the job
- The post holder will manage on behalf of the Health Board the implementation, monitoring and reviewing of NHS Funded Registered Nursing Care (FNC) and Continuing NHS Health Care (CHC) in Care Homes (Nursing) and in the domiciliary setting to ensure patients receive care of a quality which meets best practice and national standards as prescribed and funded by the NHS.
- The post holder will be expected to formally coordinate the bringing together of MDT members to undertake assessments for CHC funding within the acute, community hospitals and domiciliary setting which is consistent with National Guidance and Local Specifications (Standard Operating Procedure) for CHC funding.
- The post holder will work in partnership with the local authorities and the Independent Sector to complete joint reviews for CHC and FNC patients wherever possible.
- The post holder will provide professional nursing advice to the staff in the Care Homes (Nursing) to ensure that appropriate referrals are made to other members of the clinical team.
- The post holder will provide and deliver training packages on CHC and FNC.
The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.
