Patient Safety and Investigation Lead

An exciting opportunity has arisen for an experienced Patient Safety and investigation lead to join the Patient Safety Team at The Dudley Group
NHS Foundation Trust.
We are looking for a self-motivated, organised and dedicated individual to support divisions and directorates to manage their incidents. The
individual must have excellent communication and time management skills, able to manage a busy workload with a flexible approach and
experience in leading an investigation and report writing.
The successful candidate will play a key role in the implementation of the Trusts patient safety incident response plan helping to ensure the Trust actively learns in response to incidents and promotes a positive safety culture .
To ensure that appropriate systems and processes are in place across the Division to capture the learning from incidents, share good practice, opportunities for improved practice and actively change the delivery of care as a result of lessons learned.
To coordinate and ensure incidents are managed effectively, challenging the robustness of information provided and the effectiveness of actions
to manage the incidents.

The post holder will be responsible for the coordination of incident responses to ensure they are managed effectively and that any resultant plans are robust, implemented and lessons learnt are shared. This will include participating in investigations, working with a range of internal and
external stakeholders and development and delivery of specific training programmes to staff .

- Substantial NHS experience
- Previous experience of working with incident management within an NHS environment
- Experience of risk management
- Experience of PSIRF investigation tools [including Patient Safety Incident Investigation (PSII), Swarms and After Action Reviews (AARs)] and methodologies
- Experience of leading PSIRF investigations – including complex and multiple stakeholder investigations
- Experience of incident evaluation and assessment utilising a System Engineering Initiative for Patient Safety (SEIPS) approach
- Successful working with various levels of staff/disciplines
- Experience of formal and informal teaching (presentation skills) to small and large groups (internal and external to the Trust)
- Undertaking clinical incident investigation

Qualifications & Education

- Degree level qualification or equivalent experience within the healthcare setting

- Completion of recognised training for Patient Safety Incident Response Framework (PSIRF)
- Knowledge and ability to use databases, Outlook, Word and Excel (Microsoft office) and DATIX (or equivalent)

- Effective interpersonal and facilitation skills and supporting others

- Ability to collate and analyse complex and contentious information to determine recommendations to mitigate risk and document constructively in reports to present to meetings at local, divisional, trust wide groups or committees
