Job overview
We are seeking a dedicated and compassionate Band 5 nurse to join our out of hours community nursing team. Delivering high quality responsive care to patients in their own homes across the Sandwell and West Birmingham area.
Working closely alongside our Urgent response palliative care nurses you will play a vital part in helping patients remain at home , providing care to prevent unplanned hospital admissions.
The successful applicant will have excellent clinical assessment skills , the ability to work under pressure and a commitment to delivering compassionate care.
A flexible, responsive approach and good communication skills are also a essential aspect of this role as you are often the first point of contact for our patients overnight.
Main duties of the job
As an Out of Hours Community Nurse, you will play a vital role in delivering responsive, high-quality care to patients in their own homes overnight. This is a dynamic and varied role, supporting patients with urgent nursing needs across a range of conditions. You will manage pressure areas and wounds, support diabetes care including insulin administration, carry out clinical assessments, administer medication via a variety of routes , and provide personal care where required. You will also complete verification of expected deaths when necessary.
In addition to your independent work, you will work closely alongside the Urgent Response and Palliative Care Team to support patients requiring end-of-life care, ensuring comfort and dignity while helping them to remain at home wherever possible.
You will supervise and delegate to Band 2 Healthcare Assistants, maintaining high standards of care throughout the night. Your role will be key in delivering timely interventions that help patients avoid unnecessary hospital admissions and receive the right care at the right time.

Detailed job description and main responsibilities
To plan and implement the nursing care with the co-operation and
involvement of users and carers where appropriate.
To demonstrate, teach and advise users and carers on self-care, safety and promotion of good health by utilising all opportunities for health promotion and education.
To participate in clinical supervision.
To evaluate the condition of users and the effectiveness of care provided and initiate any further action required.
To report and, where necessary, initiate emergency action with regard to
changes in users condition or behaviour.
To give professional advice and support as necessary to nursing colleagues.
To utilise a questioning approach to nursing and have an appreciation of
relevant clinical research findings.
To assist effective communication within the clinical environment and within the Nursing Team.
To participate as directed by Band 6 and Band 7 staff in providing community experience for students and others as appropriate, and in induction/orientation programmes for learners and new staff.
To undertake other duties as and when required after consultation and
discussion with the Team Leader.
To assume responsibility for own professional conduct with regard to
confidentiality, professional standards of care, quality of service clinical
updating and accurate record keeping, both manual and computerised.
To demonstrate a consistent high standard of care in infection control and prevention.
To maintain active NMC registration and follows NMC professional
guidelines/registration.
Actively contributes to clinical audits and other audits as required.

Essential criteria
- Experience of working individually and as part of a team
- Experience in assessing, planning, implementing and evaluating care needs
Desirable criteria
- Some understanding of the workings of community services &/or intermediate care service
- Involvement in audit & research
- Additional experience outside Nursing relevant to client group eg care / volunteering
Qualifications
Essential criteria
- Registered General / Adult Nurse.
- Good standard of general education
- Registered with NMC
