Job overview
We are looking for a dynamic and enthusiastic senior clinical pharmacist who is keen to support medicines optimisation of older adults with complex disease. The post holder will be responsible for the senior oversight of the Care of Older Adults Directorate. They post holder will also participate in multidisciplinary team working on wards and in clinics, delivering high quality, cost-effective and patient-focused pharmaceutical care.
The individual will act as a clinical role model within the Pharmacy department and supervise junior and pharmacy support staff at ward level. The post holder will be responsible for the delivery and further development of pharmacy services to the Care of Older Adults Directorate.

Main duties of the job
To provide clinical and directorate support to the Care of Older Adults Directorate and wards. The directorate is located across all 3 sites at RWT.
Support the Principal pharmacists for Division 2 with developing relevant clinical services and the Principal pharmacist for Division 3 in the provision of clinical input to virtual wards.
To provide specialist clinical pharmacy advice to patients and liaise with the consultants and nursing teams to support safe and appropriate prescribing in this patient group.
To ensure that developments in the delivery of the clinical pharmacy service to patients who are treated in line with the strategic direction for the development of clinical pharmacy services within the Trust.
To continuously monitor the drug expenditure against budgets for the Care of Older Adults Directorate. To analyse expenditure information, seeking and investigating changing trends and making recommendations for savings, without compromising patient care, to appropriate consultants and departments. To assist consultants and departments in implementing the agreed changes required to make cost savings.
To manage any medicine-related switch schemes or CIPs that may arise, in conjunction with the wider pharmacy and Care of Older Adults teams ensuring as far as possible a safe, efficient and comprehensive service.
To work under the supervision of the Principal Pharmacist for Division 2 (medicine).

Qualification
Essential criteria
- Pharmacist registered with the GPhC
- Post Graduate Clinical Diploma or equivalent
Desirable criteria
- Independent Prescriber qualification or working towards this qualification (score 2 if course ongoingt)
- Additional postgraduate qualifications
Experience and skills
Essential criteria
- Significant post registrational experience
- Knowledge and experience of General and Older Adult Medicine
- Able to work in multidisciplinary teams and independently.
- Experience in supervising, training and assessing staff
Desirable criteria
- Experience working in an outpatient clinic
