Job overview

Working in a close knit team in our dedicated vascular outpatient clinic, you will be well supported by both your nursing and surgical colleagues.  

The job entails running the busy daily clinics in our gate 5 rapid access clinic.  Patients requiring urgent review/care will be referred in and seen usually within a time line of either 48 hour or 7 days.  All patients are seen in a 'one stop' capacity and are given a management plan going forward or if deemed necessary admitted to hospital.

Main duties of the job

This well supported role within a friendly and motivated team involves helping to run our daily rapid access service at Gate 5. The vascular rapid access consists of daily vascular ‘hot’ clinic which caters for patients who require urgent vascular assessment within the outpatient setting, supported by vascular nurses and the vascular surgical consultant.

The post holder will be caring for patients with a wide range of vascular conditions including; large abdominal aortic aneurysms, TIA/CVAs, acute and chronic limb ischaemia and complex wounds.

This role will also include triaging referrals, contacting patients to make appointments and providing patient advice.

Detailed job description and main responsibilities

This well supported role within a friendly and motivated team involves helping to run our daily rapid access service at Gate 5.  The vascular rapid access consists of daily vascular ‘hot’ clinic which caters for patients who require urgent vascular assessment within the outpatient setting, supported by vascular nurses and the vascular surgical consultant.

The post holder will be caring for patients with a wide range of vascular conditions including; large abdominal aortic aneurysms, TIA/CVAs, acute and chronic limb ischaemia and  complex wounds.

This role will also include triaging referrals, contacting patients to make appointments and providing patient advice.

 

In addition the post holder will also support for the diabetic foot clinic running each afternoon Monday – Thursday supported by podiatrists and a vascular Consultant.  The diabetic foot clinic provides urgent multidisciplinary care to those people with a diabetic foot wound or charcot joints.

The network’s Vascular CNS’s and rapid access clinic nurse will support you in your clinical practice and provide training. The post holder will be taught how to assess a limb using modalities such as hand held Doppler.  In addition you will learn how  to interpret ABPI, TBI and TCp02 measurements and understand different modes of imaging.

 

The post holder will be a qualified nurse with an interest in vascular surgery and wound care acting within their professional boundaries as part of the multi-disciplinary team to deliver high quality care to patients with vascular conditions.

The post holder will demonstrate safe independent practice, and following a period of training will use clinical decision making skills including assessment and diagnostic skills for patients with vascular conditions.

The post holder will be expected to work closely alongside vascular consultants, vascular clinical nurse specialists, vascular scientists, podiatrists and other members of the multi-disciplinary team.

The post holder will take responsibility for the supervision and guidance of other staff including health care assistants and students on a daily basis, actively contributing to a good learning environment.
