Job overview
We currently have an exciting opportunity to join our Nurse Assessor Team in the Cardiff S&E Locality.
We are looking for an individual who has excellent assessment and communication skills. The successful applicant will need to demonstrate the ability to work both as part of a team and also be able to work autonomously across agencies.
The post will involve undertaking assessments for FNC and CHC processes, therefore a sound knowledge of the National Framework is essential. As well as assessing patients to ensure they are receiving the right level of care in the right setting, the successful applicant will also monitor standards of care within Nursing Homes in their area and will be working closely with other agencies, such as the Local Authority, CIW and the Safeguarding Team.
Main duties of the job
Nurse Assessors are expected to:
- Undertake nursing care assessments including Continuing Health Care (CHC) assessments.
- Review Funded Nursing Care (FNC) and CHC packages.
- Have a knowledge of the newly revised CHC Framework.
- Ensure standards of care undertaken in nursing homes.
- Work in close collaboration with partner agencies, including Local Authority, nursing homes, care agencies, Care Inspectorate Wales, Safeguarding Team, palliative care colleagues.
- Source packages of care for clients in the community.
- Liaise with secondary care colleagues.
- Attend Quality Assurance panel, as well as CHC panel, and present cases for ratification.
- Make use of Electronic Patient Record systems.
- Mentor and educating colleagues and student nurses.
The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.
