Are you an experienced Clinical Nurse looking to enhance your skills and knowledge?

We have a perfect opportunity for a Clinical Nurse Practitioner/ Site Lead, to join a well-established team.

The post holder will be responsible for the operational management and clinical co-ordination of the site in and out of hours with support from the Senior Nurse Management team. 

You will provide leadership, clinical support and advice, utilising enhanced clinical skills to support both medical and nursing teams, working closely with the Senior Nurse Managers and the Hospital Head of Nursing.

Main duties of the job

The post holder is responsible for the clinical care co-ordination of the acute hospital site out of hours. Senior management advice is provided within working hours via the Senior Nurse Managers/ Acute Management team,  and via an on-call senior management rota out of hours. During the Out of Hours period, they will also have site lead responsibility with support for the on call manager.

The post holder will be expected to exercise a high degree of personal professional autonomy and make critical judgments to satisfy the expectations and demands of the role and participate in the on call rota. 

The post holder is responsible and accountable throughout the acute hospital site, to optimise patient care, patient safety, and operational efficiency.

The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.

Hywel Dda University Health Board plans and provides NHS healthcare services for people living in Carmarthenshire, Ceredigion, Pembrokeshire, and bordering counties. 

We have over 13,000 staff and together we provide primary, community, in-hospital, mental health and learning disabilities services. 

We work in partnership with the three local authorities, as well as public, private and third sector colleagues, including our valued team of volunteers. 

Our services are provided in: 

  • Four main hospitals: Bronglais Hospital in Aberystwyth; Glangwili Hospital in Carmarthen; Prince Philip Hospital in Llanelli; and Withybush Hospital in Haverfordwest 
  • Five community hospitals: Amman Valley and Llandovery hospitals in Carmarthenshire; Tregaron Hospital in Ceredigion; and Tenby and South Pembrokeshire hospitals in Pembrokeshire 
  • Two integrated care centres: Aberaeron and Cardigan in Ceredigion, and several other community settings 
  • 47 general practices (six of which are health board managed practices); dental practices (including four orthodontic); 97 community pharmacies; 43 general ophthalmic practices; and 8 ophthalmic domiciliary providers

Qualifications and Knowledge

Essential criteria
  • Level 1 Registered Nurse
  • Current NMC Registration
  • Evidence of study at Master’s level or equivalent experience with a willingness to complete MSc Advanced Clinical Practice (Nurse Practitioner) or equivalent programme
  • ALS & PLS/ complete within agreed timeframe
  • Knowledge of safeguarding issues: POVA, Mental Capacity, DoLS, Advocacy; Child protection
  • Knowledge of the sectioning of patients under the Mental Health Act
  • Knowledge of health and safety and risk management
  • Knowledge of NHS Redress and ‘Being Open’
  • Knowledge of NMC Codes of Practice
  • Maintain their personal and organisational development
  • BSc (Nursing)
Desirable criteria
  • MSc/PGDip Advanced Clinical Practice (Nurse Practitioner) or equivalent
  • Registered with the NMC as an Independent Nurse Prescriber
  • Specialist knowledge of managing patient flow/bed management
  • Knowledge of advanced trauma life support (ATNC/TNCC)
