This is a permanent new vacancy for a Hywel Dda Health board wide Adult Diabetes Insulin Pump Service
An exciting opportunity is available with the Hywel Dda Diabetes Teams to take us forward in diabetes technology and meet the requirements of NICE TA Hybrid Closed Loop.
This will include inpatients and outpatient diabetes care. The post holder will work with the multi-disciplinary team to provide specialist care, education and support for those patients and their carers using intensive insulin therapy and/or diabetes technologies- insulin pumps; continuous glucose monitoring; and Hybrid closed loops in line with National Institute of Clinical Evidence (NICE)/Joint British Diabetes Societies (JBDS) and Diabetes Technology Network (DTN) guidelines
If you have advanced knowledge of management of type 1 diabetes; carbohydrate counting; insulin dose adjustment, insulin pumps, continuous glucose monitoring and hybrid closed loops.
We invite you to apply for the Diabetes Specialist Nurse Technology Post. Extensive post registration experience of working within diabetes care is essential. Expert and advanced knowledge and experience in the management of patients with diabetes Type 1, technology in pregnancy, and supporting young adults in insulin pump therapy and hybrid closed loop systems.
Post graduate education /qualifications required in diabetes management, DAFNE, Insulin pump systems and prescribing qualification desired.
Main duties of the job
Work with the multi-disciplinary team to provide specialist care, education and support for those patients and their carers using intensive insulin therapy and/or diabetes technologies- insulin pumps; continuous glucose monitoring; and Hybrid closed loops in line with National Institute of Clinical Evidence (NICE)/Joint British Diabetes Societies (JBDS) and Diabetes Technology Network (DTN) guidelines.
- Act as a point of contact and clinical expert for patients, relatives and clinician colleagues for these technologies with the full support of the Consultant Diabetologists/Endocrinologists and the nursing team.
- Be an expert resource in insulin pump therapy including starting patients on insulin pumps and providing advice and support to patients already using an insulin pump.
- Deliver autonomous practitioner-led clinics for outpatients, inpatients both with and without an insulin pump.
- Develop and deliver patient and staff education programme in diabetes technology aligned with TREND competencies.
- Deputise and support Senior Lead Diabetes Nurse when necessary to provide clinical and managerial leadership for diabetes specialist
nursing team. - Facilitate and support patients transitioning to adult diabetes pump service on insulin pump

Essential criteria
- Experience of insulin pump initiation and follow up support
- Comprehensive experience in delivering Structured Education for patient with type 1 diabetes on an insulin pump, implementing training/teaching programmes for healthcare professionals, clients and community sector on both one-to one and group setting
- Extensive experience of working autonomously at an enhanced level within the specialist area delivering effective patient focused care
- Recent Diabetes care experience at band 6/7
- Proven experience of intensive type 1 diabetes management
- Ability to work independently and as part of the diabetes team
- Management experience
Desirable criteria
- Audit and/or research
- Advanced knowledge of evidence-based practice for the management of Type 1 diabetes and insulin pumps
- DAFNE Educator
Qualifications and Knowledge
Essential criteria
- Registered Practitioner with NMC, HCPC or GPhC
- Degree supplemented by post graduate diploma specialist training, experience, short courses plus further specialist training to masters equivalent level
- Diabetes qualification (graduate or post graduate level) supplemented by specialist training, experience and short courses ie DAFNE, Insulin Pump course
- Advanced knowledge of management of type 1 diabetes; carbohydrate counting; insulin dose adjustment, insulin pumps, continuous glucose monitoring and hybrid closed loops.
- Up to date knowledge of current clinical, professional issues and sound knowledge of current issues
Desirable criteria
- Mentorship Course / Teaching and Assessing qualification
- Counselling qualification / Motivational Interviewing
- Recognised prescribing qualification or willingness to gain within 2 years
- Quality Improvement experience
