An exciting opportunity is available for a newly qualified pharmacist or a pharmacist seeking a change of sector to join our dynamic Pharmacy team at Glangwili General Hospital.

We will support you in developing the skills and knowledge needed for the role. 

You will have the opportunity to work across a variety of general medical and surgical wards and be a key member of the multidisciplinary team, contributing to safe, effective and evidence-based use of medicines to improve patient outcomes.  

Main duties of the job

As part of a dynamic and forward-thinking pharmacy team you will:

  • Provide clinical pharmacy support.
  • Counsel patients to optimise their understanding and adherence to treatment. 
  • Work closely with multi-disciplinary teams to ensure safe and effective medicines us
  • Actively participate in board and ward rounds to promote safe prescribing practices and facilitate discharge planning
  • Support antimicrobial stewardship and medicines optimisation initiatives
  • Contribute to service development and quality improvement projects
  • Provide education and training  by supervising and mentoring junior pharmacists, pharmacy technicians, pharmacy students as well as clinical ward staff

Hywel Dda University Health Board plans and provides NHS healthcare services for people living in Carmarthenshire, Ceredigion, Pembrokeshire, and bordering counties. 

We have over 13,000 staff and together we provide primary, community, in-hospital, mental health and learning disabilities services.
We work in partnership with the three local authorities, as well as public, private and third sector colleagues, including our valued team of volunteers. 

Our services are provided in:

•    Four main hospitals: Bronglais Hospital in Aberystwyth; Glangwili Hospital in Carmarthen; Prince Philip Hospital in Llanelli; and Withybush Hospital in Haverfordwest 
•    Five community hospitals: Amman Valley and Llandovery hospitals in Carmarthenshire; Tregaron Hospital in Ceredigion; and Tenby and South Pembrokeshire hospitals in Pembrokeshire 
•    Two integrated care centres: Aberaeron and Cardigan in Ceredigion, and several other community settings 
•    47 general practices (six of which are health board managed practices); dental practices (including four orthodontic); 97 community pharmacies; 43 general ophthalmic practices; and 8 ophthalmic domiciliary providers 
•    Numerous mental health and learning disabilities services

Person specification

Qualifications and Knowledge

Essential criteria
  • Masters degree in pharmacy (4 years or equivalent)
  • Member of General Pharmaceutical Council (registered pharmacist or pre-registration pharmacy trainee during applications, must be registered pharmacist at time of start date)
  • English language qualification - GCSE grade C or above or IELTS score of 6.5 in writing and 6.0 in all other components.
  • Professional knowledge of medicines managements systems and process in an integrated NHS.

Experience

Essential criteria
  • Knowledge and experience of healthcare in a pharmacy
Desirable criteria
  • Demonstrate achievement in more than one area of medicines management
