Job overview

We are seeking a highly motivated and experienced pharmacist to join our team as a Band 8a Senior Pharmacist for Homecare and Home IV Services at Leighton Hospital.

This pivotal role supports the safe, effective, and cost-efficient delivery of medicines directly to patients in their homes, ensuring compliance with clinical and financial governance frameworks. You will work closely with the Lead Homecare Pharmacist to manage high-cost drugs, Patient Access Schemes, and Outpatient Parenteral Antimicrobial Therapy (OPAT) services, while contributing to service development, audit, and education.

As a practicing clinical pharmacist and independent prescriber, you will also provide pharmaceutical care, including medicines reconciliation, therapeutic drug monitoring, and discharge planning. You’ll liaise with multidisciplinary teams across primary and secondary care to ensure seamless patient pathways and continuity of prescribing. This role offers the opportunity to influence service innovation, mentor junior staff, and contribute to national and regional forums.

Main duties of the job

  • Help to coordinate day-to-day management of homecare services across the Trust.

  • Ensure compliance with governance frameworks and procurement legislation.

  • Monitor KPIs, investigate incidents, and report findings to MDTs.

  • Screen homecare prescriptions for safety, funding, and clinical appropriateness.

  • Manage high-cost drugs and Patient Access Schemes with accurate data tracking.

  • Liaise with commissioners and clinicians to ensure appropriate prescribing.

  • Support OPAT service development and antimicrobial stewardship.

  • Attend and contribute to weekly OPAT MDT meetings.

  • Provide clinical pharmacy services on wards, including medicines reconciliation and care planning.

  • Actively prescribe within scope to optimise patient outcomes.

  • Train and supervise pharmacy and nursing staff in homecare and IV services.

  • Lead audits, patient surveys, and contribute to policy development.

  • Deputise for the Lead Homecare Pharmacist in forums and management tasks.

  • Support recruitment, appraisals, and performance management of team members.

  • Promote safe systems of work and compliance with infection prevention policies.

  • Maintain professional standards and contribute to continuous service improvement.

Essential criteria
  • Vocational Masters degree in Pharmacy and Registration with GPhC
  • Post graduate dipolma in clinical pharmacy or equivalent experience
Desirable criteria
  • Non-medical prescribing qualification

  • Significant post registration hospital experience or equivalent documented clinical experience, with a considerable period of this being at a band 7 level or equivalent
