Job overview

We are looking for motivated Band 5 Physiotherapist to join our dedicated team here at CCICP SMASH Community Therapy Team. The successful applicant  will join our rotational programme possibly starting with the SMASH Community Team but will include various rotations to other community teams and services provided by CCICP

We have a fantastic team of highly skilled physiotherapists and look forward to welcoming you to our team. We have opportunities for education and development within our care community and across the five care communities based in Nantwich, Sandbach,  Crewe, Winsford and Northwich.  We offer a supportive induction, supervision and mentorship programme.

The community physiotherapist will need to assess, manage and provide treatment to a caseload of service users with complex and/or multiple needs. You will be working closely with the MDT who will provide supervision to build your experience alongside community therapy staff and assistants. There may be an expectation to work between multiple sites in the organisation.

The physiotherapist will be expected to join the 7-day, service and may be lone working within lone working policy.

Potential rotations may include D2A unit,  Community therapy team, Paediatrics , MSK out patients, Pulmonary team , Wheelchair services and MacMillan Service.

 

Main duties of the job

We provide a rapid, unplanned and planned response, we facilitate early discharge from hospital and aim to reduce hospital admissions. We also cover discharge to assess beds and provide therapy to Nursing Homes.

There is a varied caseload and you will need to demonstrate knowledge, experience, and relevant clinical skills with patients with complex / long term conditions, falls, orthopaedics, general mobility problems and neurological conditions.  

The successful candidate will work as a member of our integrated multidisciplinary team alongside District Nurses, Advance Nurse Practitioners, Complex Case Practitioners, MacMillan Nurses. 

There will be associated administrative duties and you will need to be computer literate with a good knowledge of Microsoft packages and have accurate keyboard skills. Our team utilises digital technology to support remote working and accommodate a flexible working approach where possible and is supported to do this with access to a laptop and mobile phone. 

The candidate will need to have experience of working in the NHS either as a student or qualified physiotherapist.  

There may be a requirement to join the urgent care response rota once competencies have been gained, with the full support of the senior Physiotherapists.

You are required to be a car driver and have access to your own vehicle.

Detailed job description and main responsibilities

Job Purpose - To undertake assessment of patients , developing your skills and knowledge within a supportive environment. To work as an autonomous clinician, under the guidance of Clinical Specialist Physiotherapists as required, using accurate analytical skills and clinical reasoning to interpret assessment findings, making diagnoses where appropriate, identifying patient problems, setting goals, and providing an appropriate treatment plan and progression.
