You will maintain an active caseload and reduce the risk of patients re-attending A&E due to violence-related injuries.
Main duties of the job
You will utilise a multi-agency approach to improve the outcomes and life chances for children and young people between the ages of 10 and 25, with an initial focus on ages 10 to 17, who are experiencing multiple problems, including serious violence, to reduce the impact upon emergency services and local communities.
Form strong professional relationships with the Children and Young People's Emergency Department and Urgent Treatment Centre teams and work closely with them to identify patients who may benefit from the Navigator service.
Responsible for engaging new individuals into the service by working closely in partnership with treatment and recovery providers and with the recovery community.
