Job overview
This if fixed term for 6 months (possibility of secondment for internal staff but you must obtain approval from your manager).
This is an exciting opportunity to develop your skills within stroke rehabilitation. The Hambleton & Richmondshire ESD team is a relatively newly established yet forward thinking and supportive team, comprising of a Consultant Therapist, Speech and Language Therapist,
Physiotherapist, Neuropsychologist, Dietitian and Assistants. We work closely with the rehab team to ensure timely discharge from hospital and continue the rehabilitation journey in the patient's home. We carry out joint visits as required, and have weekly MDT meetings to discuss progress of each patient. We utilise outcome measures and patient feedback to ensure we are delivering the best service possible.
Successful candidate must have access to a vehicle for work purposes to meet the requirements of the role.
Main duties of the job
To provide specialist occupational therapy assessment, management and treatment of stroke patients within their own home, following their discharge from hospital, in order to regain/maintain their independence.
To devise and deliver specialised programmes of care for Stroke patients and communicate treatment details to patients and caregivers.
To work closely with other members of the multi-disciplinary (MDT) team to provide a holistic therapeutic approach to functional rehabilitation
To support Therapy Assistants to follow a programme of care to ensure responsive and effective interventions

Qualifications
Essential criteria
- To hold a Diploma or Degree in Occupational Therapy
- To be registered as an Occupational Therapist and fit to practice by the Health and Care Professions Council (HCPC)
- To have achieved or be willing to work towards relevant diploma level or equivalent post-graduate qualification
Experience
Essential criteria
- Able to work both as an autonomous practitioner and as part of a team
- To be able to travel independently throughout the localities
- Ability to work across professional boundaries working with multiple teams of healthcare professionals in more than one location
Knowledge
Essential criteria
- Can demonstrate relevant/varied post graduate experience including treating patients with stroke
- Knowledge of relevant Stroke national guidelines and standards
- Evidence of audit and participation in service development
- To demonstrate analytical skills and judgement
- Ability to work to unpredictable work patterns on a frequent basis e.g. working to deadlines, interruptions
