Job overview

If you have been working as a Band 5 Dietitian in a community  environment,  and are looking to move to the next step, this role might be the one for you. We are looking for dietitian to join our community team at Chesterfield Royal Hospital providing dietetic  services,  mainly by telephone to patients at home and in care homes. The post will also involve seeing patients face to face at home, in care homes and in our community hospitals. Whilst the majority of the caseload will be oral nutrition support the post holder will, with support, get the opportunity to review patients who are enterally fed.

Whilst the team are based at Chesterfield Royal Hospital and there will be a requirement to come site for 50% of the time the post holder will be able to work from home.

Detailed job description and main responsibilities

Clinical.

1.

To provide a client centred nutrition & dietetic service to individuals and their carers at locations across the health community.

2.

To participate in the team approach to the care of individuals referred to the service.

3.

To liaise with the multidisciplinary team across the health community to ensure the nutritional needs of individuals can be met.

5.

To accurately collect relevant activity data required for service monitoring, management and evaluation.

6.

To develop evidence based dietary information in conjunction with other members of the community team.

7.

To provide cover for dietetic colleagues as appropriate.

8.

To provide clinical knowledge and expertise to the multidisciplinary teams based on sound knowledge of evidence based practice.

Operational Management.

1.

To manage and support Community Nutrition & Dietetic staff.

2.

To work with the team leader to ensure adequate cover arrangements are in place to deliver effective nutrition support services are provide based on clinical need.

3.

To identify and implement service developments within the community to ensure service objectives are met and to support the innovation and transformation agenda.

 Clinical Governance.

1.

To be actively involved in the implementation and development of Clinical Governance.

2.

To advise the Head of Nutrition and Dietetic Service and the Community Team Leader on professional issues relating to community nutrition support.

3.

 

To work with the community dietetic team and the Community Team Leader to ensure the nutrition and dietetic training needs of nursing, medical and other staff are met within the health community.

 4.

 To contribute with other members of the nutrition and dietetic team to the identification,  development and implementation of standards of dietetic practice.

 5.

 To represent the nutrition and dietetic service on community       forums within the North  Derbyshire health community.

 6.

 To actively participate in clinical audit and research.

 7.

 To participate in and support other staff with clinical supervision.

 8.

 To demonstrate a commitment to CPD by maintaining an up to     date professional portfolio.

 9.

 To provide support and participate in the training of student     dietitians and be the  Dietetic Practice Placement Lead for the   community service.

10.

To take the lead within the service with the other Band 6 Dietitian working with  community hospital catering staff to develop menus that meet the requirements of the Healthcare Food Standards.

  • HCPC registered

Experience

Essential criteria
  • Significant dietetics Band 5 experience in an NHS community setting
  • Significant experience of working with individuals requiring nutrition support (including enteral feeding)
  • Involvement in student training
  • Teaching experience
  • Healthcare menu development and nutritional analysis

  • Excellent written and oral communication skills
  • Ability to work as both as an autonomous practitioner and as part of a team
