As a Care Coordinator in the dynamic Joined Up Care Derbyshire ‘Care Transfer Hub’, you will be at the heart of a team dedicated to processing intermediate care referrals.

Your role will be pivotal, acting as the essential link between service users, carers, primary care, secondary care, community care, social care, mental health services, out of hours services, and voluntary organisations.

This is more than just a job; it’s an opportunity to connect with a diverse network of professionals and truly contribute to the well-being of individuals and the community.

Main duties of the job

Monitoring a caseload within the Care Transfer Hub to track a person's journey and outcomes - escalating delays or concerns to the multi-disciplinary team. 

Chasing additional information and updates to referrals, ensuring this is documented in the shared database. 

Liaising with organisations across the Joined Up Care Derbyshire system to identify service availability and provide updates. 

 

Working for our organisation

The Care Transfer Hub is a newly developed service to facilitate timely access to intermediate care. This role will support the hub team to navigate, coordinate and plan care for the people of Derbyshire. This is an exciting time to join the team and be involved in the development of the Care Transfer Hub and its processes. There are opportunities to work flexibly and the role will require you to be adaptable and open to change.

Qualifications

Essential criteria
  • Diploma, Higher National Certificate, Level 4 Qualification (or relevant experience)
  • Competent in using IT systems including SystmOne and Microsoft Office
  • Willing to undertake ongoing internal and external training to keep up to date with changes and developments
Desirable criteria
  • NVQ Level 3 Business Administration

Experience

Essential criteria
  • Minimum of 2 years' experience of working with healthcare professionals/within health and social care
  • Experience of using databases
