Job overview

An exciting opportunity has arisen to join our dynamic Virtual Ward (VW) team, supporting a hospital-at-home approach for patients across Eastbourne and Hastings.

Our Virtual Wards provide a fast-paced and innovative service, allowing patients to receive high-quality care in the comfort of their own homes. This helps to prevent unnecessary hospital admissions and enables earlier, safe discharge from hospital settings.

As a Patient Coordination Administrator, you will play a vital role in the smooth running of the service. You will be responsible for:
✔ Coordinating and managing virtual ward caseloads.
✔ Supporting remote patient monitoring clinicians.
✔ Handling digital documentation and ensuring records are up to date.
✔ Liaising with patients, families, and colleagues to facilitate effective communication.
✔ Providing general administrative support to ensure the efficiency of the service.

We are looking for an organised, proactive, and compassionate individual with strong administrative skills and the ability to work in a busy healthcare environment. Experience in a healthcare or customer-facing role is desirable but not essential.

Main duties of the job

As a Patient Coordination Administrator, you will play a vital role in ensuring the smooth running of the Virtual Ward service. Your main duties will include:
Remote Monitoring Management – Coordinating the setup and delivery of remote monitoring devices and recording patient observations from the dashboard.
Patient Visit Coordination – Supporting clinical teams by scheduling and managing patient visits.
Communication & Documentation – Handling calls and emails, liaising with patients, families, and clinical staff, and ensuring accurate documentation of meetings.
Administrative Support – Ordering and organising supplies, maintaining records, and assisting with day-to-day operational tasks.
Team-Based Working – Based in the Virtual Ward Hub at Hampden Park Health Centre, Eastbourne, working collaboratively to ensure efficient service delivery across the Virtual Ward and Urgent Community Response combined workforce.
Weekend Working – This role includes weekend work as part of a rota to ensure continuous patient support.

Qualifications

Essential criteria
  • Educated to GCSE level or equivalent including Maths and English or equivalent experience
  • NVQ level 3 in Administration / Secretarial or equivalent level of knowledge and experience
Desirable criteria
  • Recognised to computer / typing qualifications, e.g. RSA III, NVQ 3 or equivalent.

Skills

Essential criteria
  • General up to date office procedures
  • Knowledge of Microsoft Word, Excel, PowerPoint and Access
Desirable criteria
  • Knowledge of Health and/or social care

Experience
