Job overview

The post holder will be responsible and provide effective leadership for a clinical team providing a high quality service within the resources available within a defined area, and specifically to deliver on the following key areas:

To co-ordinate and lead team functioning with particular focus on:

  • Management of referrals including delivery of care
  • Performance monitoring and management
  • Line management and supervision of staff

The post holder will contribute to facilitating the transformation of community mental health team in line with South Essex Mental Health Strategy.

Main duties of the job

  • Manage and lead the multi-professional team in the delivery of safe and effective evidence-based interventions
  • Hold a small caseload
  • To deliver a service within the agreed financial envelope ensuring maximum value in term of clinical and cost effectiveness
  • Ensure that a culture of effective risk management is present in the team
  • Work in conjunction with the service Managers and Directors to ensure that access targets are achieved
  • Working collaboratively to ensure that appropriate rota cover is in place, including Approved Social Work cover
  • Allocate cases, and monitor practitioners’ workload through effective implementation of the workload zoning tool
  • Organise and chair Enhanced Care Planning and Review meetings for clients who present with significant risks and who have complex needs
  • Organise and chair weekly MDT meetings jointly with the Lead Team Consultant ensuring required records are maintained
  • Lead and ensure the implementation of agreed action plans including risk management plans for individuals with high risk/complex needs
  • Contribute to effective communication and liaison within the wider MH services and with other services and professionals including statutory and voluntary agencies and community groups
  • Act as Designated Officer for Adult Protection Investigations
  • Ensure service information requirements are met through the production of required statistics and data, and completion of daily diary sheets

Here at EPUT we maintain our own bank of specialist staff to ensure that we are able offer safe and effective care at all times.

Detailed job description and main responsibilities

  • Provide regular supervision and support to all staff that will include monitoring of caseloads and clinical practice. Supervision will also include reviewing standards of record keeping such as appropriate updating of assessments, care plans, risk management plans and carers assessments.
  • Act as a member of the Mental Health Management Team and contribute to the development of policy formulation, planning and resource allocation.
  • Assist in the investigation of complaints and incidents under the direction of the Community Mental Health Team Manager and or Service Manager/Associate Director.
  • Lead multidisciplinary / multiagency practice development and service improvement projects.
  • Ensure that the Service User and Carers are actively involved in the planning and delivery of services in keeping with the Trusts engagement policies.

  • RMN/equivalent professional qualifications
  • Qualification at degree level or equivalent
  • Evidence of continuing professional and personal development
  • Knowledge of Safeguarding issues and child protection requirements/legislation and how these apply to the role
  • Ability to assess clinical situation quickly and use initiative
  • Experience in of working closely and developing relationships with senior medical nursing, social and clinical staff
