Main duties of the job

We are pleased to offer the new and exciting opportunity to join the new Mid and South Essex, Older Adult Admission Avoidance & Home Treatment Team.  As part of the transformation agenda a new service is being formed to provide support to Older Adults, with Frailty and Functional Mental Health needs.

Here at EPUT we maintain our own bank of specialist staff to ensure that we are able offer safe and effective care at all times.

All our permanent staff are automatically enrolled onto the staff bank however this does not mean you have to work any additional shifts, but the option is there for you if you wish.

Detailed job description and main responsibilities

As part of this developing service you will help support older adults with functional mental health issues to meet increased needs / rising risk to support admission avoidance and provide home treatment with a Home First approach to support. You will work with a new and growing team, the existing community services and the established Dementia Admission avoidance services across Mid and South Essex

 

Person specification

Education/Qualifications

Essential criteria
  • Registered Mental health Nurse, or Occupational Therapist.
  • Additional Professional Qualifications in Older adult Mental Health /Dementia or Experience
  • Evidence of personal and professional development in contemporary mental health practice and dementia care
Desirable criteria
  • Educated to Degree Level
  • Non-medical Independent prescriber

Knowledge

Essential criteria
  • Knowledge of relevant legislation and guidelines.
  • Good understanding of the Mental Capacity Act and its application in practice
  • Understanding of the Care Act
  • Good understanding of national policy and the implications for good mental health
  • An understanding of Frailty and the complexities of later life
  • Experience in the use of risk assessment tools
Desirable criteria
  • Use of screening tools in dementia

Skills/Experience

Essential criteria
  • Evidence of ability to prioritise workload and competing demands
  • Evidence of objective setting, time management skills etc
  • Ability to plan ways of improving efficiency and cost effectiveness of clinical services
  • Evidence of personal development
  • Ability to build relationships and negotiate satisfactory outcomes
  • Ability to know when to seek the support and guidance of more senior colleagues when necessary
  • Ability to use own judgement confidently and decisively
