This is a community-based role with your duties being carried out between your home, community setting,
We work across the whole of East Sussex (except for Brighton & Hove) and have 1 training post available.
Main duties of the job
Your daily activities will be varied to include:
- promoting public health initiatives. to support Healthy lifestyles for Children and Young people and their families.
- delivering packages of care e.g. healthy weight, toileting, behaviour/ parenting, emotional health and well-being.
- undertaking health needs assessment
- providing Support in the daily management of Young people's texting service
- managing a caseload of Children and Young People
- focusing on high impact areas set out in the healthy child programme
- safeguarding of children and families
- leading on QI projects

Qualifications
Essential criteria
- Registered Nurse or Midwife with current NMC PIN, Level 6 Degree or equivalent qualification
Experience
Essential criteria
- Minimum of 1 years’ experience working as a Nurse or midwife
Knowledge
Essential criteria
- Sound communication skills in order to understand and communicate both verbally and in writing
- You will have a client focussed approach which is essential to provide patients, clients and their families with the best possible experience
Desirable criteria
- Having an awareness of developments within the NHS would be an advantage.
Skills
Essential criteria
- Able to use IT systems and databases
- You will have knowledge of up to date clinical practice by using up- to- date skills and adhering to evidence-based procedures and clinical effectiveness frameworks.
Abilities
Essential criteria
- Ability to manage time effectively, priorities and work under pressure to meet deadlines both academically and in practice, adaptable to an ever changing and challenging work environment. Ability to work effectively with a variety of people at all organisational levels across different sectors including areas of social deprivation
Working environment
Essential criteria
- A full, valid UK driving licence and the ability to travel independently are essential for this role
