An exciting opportunity has arisen for a band 6 Cardiac Rehabilitation Specialist Nurse to join the team at East Lancashire NHS Trust.

You will be joining an accredited service delivering cardiac rehabilitation to patients who have had a cardiac procedure, event or a certain cardiac conditions such as heart failure. 

Main duties of the job

 

     Your role will be to provide early phone consultations with patients, offering reassurance and support, delivering face 2 face community clinics, observing and participating in the exercise classes and education sessions. Working flexibly to cover clinics at short notice to meet the needs of the service . Supporting patients in accordance with the British Association of Cardiovascular Prevention and Rehabilitation (BACPR), and the "NHS Commissioning standards for cardiovascular rehabilitation", adopting a personalised approach to patient care that maximizes independence, choice and control, enabling people to reach and maintain their optimum level of health and well being.

Detailed job description and main responsibilities

·        Work as part of an integrated team working to improve the care and lives of patients with Acute Coronary Syndromes, interventions, Heart Failure and other cardiac conditions.

·         Work within an integrated Specialist Cardiac team supporting other members of the MDT

·         Work alongside the Cardiac Rehabilitation Nurses and administrators to deliver the Cardiac Rehabilitation Service.

·         Participate in the running Cardiac Rehab classes.

·         Be a support to the other team members students and HCP.

              Aim to maintain the National Audit of Cardiac Rehabilitation                              accreditation.

Qualifications

Essential criteria
  • Registered Nurse with current professional NMC registration
  • Evidence of continuing professional practice (CPD)
  • Evidence of recent study
Desirable criteria
  • Degree or working towards
  • Cardiology module
  • Counseling skills certificate
  • NMP qualification or willingness to work towards
  • Exercise qualification

Experience and Knowledge

Essential criteria
  • Substantive experience of nursing within a cardiology setting
  • Experience of implementing research into practice
  • Local and national audit
  • Up to date knowledge of cardiac rehabilitation
  • Knowledge of cardiac procedures and conditions including heart failure
  • Awareness of the Commissioning standards for Cardiovascular Rehabilitation
  • Knowledge of the BACPR Standards and Core Components
  • Evidence of introducing new concepts and projects
  • Basic IT skills in word, email, EPR
Desirable criteria
  • Cardiac Rehabilitation experience
  • Previous experience of community working
  • Previous experience of partnership working with other organisations
  • Motivational interviewing training

  • Able to maintain clear and accurate patient records
  • Excellent communication skills used in a variety of settings with patient’s, relatives and carers with varying levels of understanding
  • Proven ability to develop effective interpersonal relationships with colleagues in health care settings

  • Proven ability to manage priorities under pressure
  • Ability to adapt to change and participate in service development

Other Requirements

Essential criteria
  • Willingness to work flexibly on both Blackburn and Burnley sites when required, including own transport
  • Willing to undertake necessary training to fulfil the role of the cardiac rehabilitation specialist nurse
