Job overview

Due to career progression, an exciting opportunity has arisen for an enthusiastic and motivated pharmacist with an interest in Cardiology to join our Clinical Pharmacy team.

This role offers the opportunity to develop and consolidate specialist clinical knowledge within cardiology services, working closely with multidisciplinary teams to optimise medicines use and improve patient outcomes. You will provide expert pharmaceutical care to patients across a range of cardiology and cardiothoracic specialties, supporting the safe, effective and evidence-based use of medicines.

As part of a supportive and experienced pharmacy team, you will work alongside Lead Pharmacists, consultants, nurses and other healthcare professionals to deliver high-quality clinical pharmacy services. You will contribute to service development, clinical governance, medicines optimisation initiatives, education and training, and departmental quality improvement projects.

This post is ideally suited to a pharmacist looking to gain or further develop specialist cardiology experience and build the skills required for future senior clinical and leadership roles. We are looking for an individual with drive, enthusiasm and a commitment to delivering excellent patient-centred care.

Main duties of the job

You will apply your clinical knowledge, skills and experience to optimise patient care with cardiovascular conditions, ensuring safe, effective and evidence-based use of medicines across cardiology and cardiothoracic services.

You will work closely with consultant cardiologists, cardiothoracic teams, specialist nurses and the wider multidisciplinary team to promote high-quality prescribing and medicines optimisation through the development and implementation of clinical guidelines, protocols and service improvement initiatives.

The post holder will support the Lead Cardiology Pharmacist in the delivery and development of specialist pharmacy services, contributing to clinical audit, quality improvement projects, governance activities and implementation of national and local priorities for cardiovascular care.

There will be opportunities to work collaboratively with other specialist services, including critical care, cardiac rehabilitation, heart failure, anticoagulation and cardiac surgery teams, supporting multidisciplinary approaches to improve patient outcomes and service delivery.

You will provide specialist clinical pharmacy services to cardiology and cardiothoracic areas, undertaking medicines reconciliation, prescribing support, medication review, patient counselling and discharge planning. In addition, you will contribute to education and training programmes for pharmacy, medical and nursing staff and undertake other pharmacist duties as required by the department.
