Job overview

An exciting opportunity has arisen for an experienced pharmacist to join the innovative and friendly Anti-infectives Pharmacy Team at
London north West  University NHS Healthcare Trust. We are looking for an individual with a passion for Antimicrobial Stewardship (AMS) and  Outpatient Parenteral Antibiotic Therapy (OPAT). The majority of your role will be dedicated to the Virtual Ward and Early Supported Discharge service for the acute infection pathway but will also include working alongside other members of the Pharmacy Anti-infective Team to provide a service to the infectious disease department and carrying out the Trusts AMS initiatives.

Main duties of the job

• To lead, deliver, develop, and evaluate clinical pharmacy services to the virtual ward and early supported discharge (ESD) service for
the acute infection pathway in accordance with the objectives and priorities agreed between the post holder, Chief Pharmacist, Lead
Anti-infective Pharmacist and the leads for the virtual ward and ESD service, in order to ensure safe, clinically effective, and cost
efficient use of outpatient parenteral antimicrobial therapy (OPAT) and other anti-infective therapy including prescribing services when
required.
• To provide a consistently highly specialist pharmaceutical service for patients seen within the virtual ward and ESD service on the acute
infection pathway in line with Trust and national standards in order to meet all Clinical Governance requirements.
• To lead on Medicines Management for the virtual ward and early supported discharge (ESD) service for patients on the acute infection
pathway.

Essential criteria
  • Diploma in Clinical Pharmacy Practice or equivalent experience
  • Pharmacist GPhC registration
Desirable criteria
  • Registered independent prescriber

Experience

Essential criteria
  • Extensive post registration experience in hospital pharmacy
  • Experience within OPAT / anti-infective specialities
  • Successful collaborative working in a multidisciplinary team
  • Experience of providing training, mentorship and supervising staff
Desirable criteria
  • Leading a team of staff to deliver a service
  • Experience of MDT audit and/or research & development
