Job overview

As a Critical Care Outreach Nurse you will support the Trust in the delivery of effective and responsive care in the transition from Critical Care to ward level care.  You will be responsible for supporting the team with the focus on early recognition of deterioration and timely escalation of care whilst supporting the development of critical care skills within the Trust. 

 

 

Main duties of the job

Do you have the skills and desire to be part of our dynamic Critical Care Outreach service at London Northwest University Hospitals  NHS Trust.  Our Critical Care Services are provided across three hospital sites.  The Northwick Park Hospital Intensive Care Unit, Ealing Intensive Care Unit and Central Middlesex Hospital Enhanced Care Unit. We also have a Critical Care unit, Critical Care Follow Up Service and Advanced Critical Care Practitioners that offers opportunity for further professional growth and career development in Critical Care Nursing. This is a tri-site Trust the post holder will be required to work cross site at Northwick Park Hospital, Ealing Hospital and Central Middlesex Hospital.

 Critical Care units are specially staffed, equipped and designed to closely monitor and treat patients with life-threatening conditions.  The Critical Care Units specialise in providing a high standard of care for medical and surgical patients undergoing a variety of procedures. We care for critically ill patients with multi-organ failure; Patients with acute and chronic renal disease; Patients with infectious disease; Complex Post-operative patients, Colorectal patients, Head and Neck and ENT patients, Oncology patients, Patients with Stroke and other Neurological conditions and level 2 Trauma patients.  In addition, our Northwick Park ICU has an integrated Intermittent Haemodialysis Unit where patients from all wards and departments of the hospital are dialysed.

Detailed job description and main responsibilities

Main duties and Responsibilities:

  • Accountable for own actions in accordance with Code of Professional Conduct. Will contribute towards corporate objectives, acting within local, Trust and statutory guidelines and policies at all times.

 

  • Highly developed communication skills enabling maintenance of high quality service and ensuring timely management and resolution of unexpected events and situations and conflicting priorities

 

  • Coordinate the care of critically ill patients through the critical care pathway to ensure the effective management of their patient group.

         

  • Responsible for identifying and assessing complex clinical and non-clinical issues, implementing resolutions and monitoring/auditing outcomes.

 

  • Undertake advanced health assessment using history taking skills and advanced physical assessment knowledge to ensure optimum patient management including; patient monitoring; equipment management and defined expanded clinical roles.

 

  • Proactively develop relevant treatment plans for patients requiring intervention including fluid resuscitation, oxygen therapy in accordance with patient group directives along with the ordering of appropriate laboratory and radiology investigations. Facilitation of discharged patients from critical care to support continuing recovery, coordinating all aspects of their care with the multi-disciplinary team. The management and coordination of patients discharged from critical care to wards with a tracheostomy to include weaning and decannulation.
