Job overview
This is an exciting opportunity for a motivated and innovative operational lead to join our Integrated Discharge Team.
Are you passionate about ensuring patients are discharged in a timely way, focusing on flow and improvement of patient journey and pathways, if so - this is the ideal role for you.
We are an Integrated Discharge Team looking for a Lead to enable us to continue working to a Discharge to Assess model ensuring that patients who no longer meet the Criteria to Reside can be safely transferred to the community.
You will be working in a team that is integrated across the acute and a community, with partnerships across Brent, Harrow and Ealing, where the service is motivated to ensure that patients should be returning home as soon as they are able. Ethos of the team is 'Why not home, why not today'
Main duties of the job
Deputy head of Integrated Discharge Team, managing a team of service managers, Discharge Coordinators alongside community case managers and the adult social care team working jointly to ensure that patients are discharged in a timely and safe way.
Operational support to the Integrated Discharge Team to deliver patient flow in a timely way, including managing complex discharge plans and family expectations to avoid an extended stay in hospital.
Identification of improvements following data analysis and critical knowledge around discharge barriers.
Lead on the development and embedding of shared care planning and trusted assessments through joined up working and closer integration of services across acute, community, social care, mental health services, and third sector provision of an integrated and co-ordinated efficient and clinically cost effective care wrapped around a patient’s needs to improve wellbeing and health outcomes.
To provide strategic oversight of the discharge to assess process ensuring that there are no delays to discharge and that a “home first” approach is adopted. Including, supporting wards with education and training around the discharge once they no longer meet Criteria to Reside.

Essential criteria
- Educated to master level or equivalent experience
- Evidence of ongoing professional development
- Evidence of management development
Desirable criteria
- Registration/ qualification in Nursing/other Health profession.
Knowledge & Experience
Essential criteria
- Evidence of effective design and management of patient flows and pathways
- Experience of managing and motivating teams
- Experience of effective performance and budgetary management
- Strong leadership skills · Expert knowledge within the specialist field gained through post graduate training and practical experience and recognised by colleagues / peers, and others involved in patient care.
- Understanding of D2A/discharge pathways and future plan.
- Experience of leading and implementing service improvement including complex change management programmes
Desirable criteria
- Evidence of successful management of clinical services in the acute sector
- Knowledge of NWL patient flow agenda
- In-depth practical knowledge of Understanding of NHSE hospital support guidance
Skills
Essential criteria
- Highly developed clinical/operational skills in complex discharge planning
- Ability and confidence to communicate effectively with Requirement Essential Desirable and influence staff at a senior level
- Ability to undertake audits and analyse complex information related to patient flow
- Ability to deal with conflict and stressful situations and to problem-solve and take decisions
