Job overview

The aim of Adult Community Services is to provide a first-class healthcare service across the in-patient and community services. We provide services to a broad multicultural community throughout Hackney and the City.  We have an excellent training, development and support mechanism in place to enable our staff to meet the high standards of healthcare that the residents of Hackney and the City expect and deserve.

We have Deputy District Nurse Team Leaders vacancy within the Adult Community Services Department to provide nursing care to allocated patients within their homes.

Main duties of the job

The post holder will be responsible for the planning, implementation and evaluation of nursing care needs and the delivery of high quality and up to date care to allocated patients in their own homes and/or a variety of settings.

The post holder will work within a team for a defined number of patients, and will work under the supervision of the Clinical Operations Manager. You will be responsible and accountable for the management of a Caseload in the absence of the Clinical Operations Manager/ District Nurse. You will also work closely with the aligned GP’s to your team and will be both the advocate and the key chain of communication for their care.
