Trainee Frailty Advanced Clinical Practitioner

Job overview

 Our Integrated Frailty Team are expanding, and we want you to be part of our next development! Due to continued investment within the team we’re recruiting Trainee Advanced Clinical Practitioners. This exciting new role is a fully funded training position, you will be supported through a 2 year master’s degree programme to compliment your professional development in Frailty,  resulting in progression to band 8a upon completion of the course. You will attend university and have  additional study time each week and then spend the remaining days/hours working in the Integrated Frailty Team.  As a team we believe in patients receiving the right care, in the right place, at the right time and will support our patients to safely avoid an admission to hospital wherever possible. This is a 7 day service , therefore you will require a flexible approach to working hours. 

Main duties of the job

Duties will include but not limited to:

• To practice autonomously with a high level of accountability at an advanced specialist level in the direct delivery of high quality care.

• Uses advanced therapeutic clinical skills to effectively assess and manage frail patients presenting to the Trust.

• Plan and manage complete episodes of care, working in partnership with others and delegating and referring as appropriate to optimise health outcomes and resource use, as well as providing direct support to patients and clients.

• To provide clear leadership and direction to support the Frailty Therapy Team and wider MDT, acting as a role model for the Trust, presenting a positive image to patients and staff.

• Advocate and contribute to a culture that supports continuous learning and development, evidence-based practice and succession planning.

• Critically engage in research activity, adhering to good research practice guidance, so that evidence based strategies are developed and applied to enhance quality, safety, productivity and value for money.

Qualifications

Essential criteria
  • Registered with professional body (HCPC/ NMC/GpHC). Relevant professional qualification
Desirable criteria
  • Teaching qualification Experience of working with older adults

Training and Experience

Essential criteria
  • Experience in a senior clinical position delivering high quality patient centred care.
  • Portfolio of evidence demonstrating achievements in practice/ speciality
  • Experience within an acute care setting
  • Quality improvement experience
Desirable criteria
  • Evidence of education relating to leadership skills development.

KNOWLEDGE

Essential criteria
  • Academic capability and commitment to undertake an MSc in Advanced Practice
