Bank Clinical Pharmacist - Band 7

We are looking for enthusiastic pharmacists  to join our growing bank pharmacy team for working anything  up to 37.5 hrs a week.  Your base will be one of the 2 main sites- the main ones being  Aintree or Royal Liverpool Hospital

Main duties of the job  include providing a clinical pharmacist service to nominated wards within Liverpool Univeristy Hospitals NHS Foundation Trust (LUHFT). This will require working across a range of specialities including Emergency care, medical and surgical wards, taking medication histories and reviewing mediciation to optimise patient care .

Main duties of the job

  • Undertaking medicines reconciliation on admission.
  • Review medication to ensure safe and effective use.
  • Formulation of individual pharmaceutical care plans.
  • Review and update of care plans depending on response, results of
    investigations and adverse effects.
  • Therapeutic drug monitoring for specific drugs.
  • Educate patients about their drug therapy.
  • Develop evidence based treatment protocols for use either within the directorate or across the Trust where appropriate.
  • Facilitate implementation of local and national guidelines where appropriate.

Detailed job description and main responsibilities

  • Provide specialist pharmaceutical information to medical and nursing staff
    and patients on the designated ward(s) and to General Practitioners and
    other staff within the Trust where appropriate.
  • Participate in consultant ward rounds and multidisciplinary meetings relevant to the designated ward(s).
  • To participate in the therapeutic drug monitoring service provided in
    accordance with departmental procedure.
  • Undertake clinical audit and practice research within the Pharmacy
    Department.
  • Record regularly clinical interventions and activities in line with departmental policy.
  • Provide professional and legal clinical supervision and act as the designated
    Responsible Pharmacist in the dispensary as allocated.
  • Prepare the weekly pharmacist dispensary cover rota in line with
    departmental policy.
  • Investigate any medicines related incidents or complaints from the designated clinical area and explore ways of minimising risk of future re-occurrence or
    risk to staff.
  • Liaise with appropriate health care professionals within Primary Care Trusts
    to ensure the effective and efficient use of resources across the
    primary/secondary care interface and where necessary develop shared care
    protocols.

Qualifications

Essential criteria
  • Vocational masters degree in pharmacy
  • Membership of The General Pharmaceutical Council
  • Diploma in clinical pharmacy or equivalent documented clinical experience
Desirable criteria
  • MRPharmS

  • Delivery of clinical pharmacy services to a range of specialties
  • Commitment to continuing professional development / education
