Detailed job description and main responsibilities
· Develop and implement safe and individualised exercise programmes for patients with
cardiac conditions.
· Provide home based exercise prescription to individuals choosing home based
rehabilitation, to be able to monitor and progress.
· Set appropriate exercise-based goals with patients and facilitate return to work or activities.
· Teach cardiovascular and strength training exercises individually and as part of the group
sessions.
· Deliver the warm-up and cool-down session to the group and monitor patients during the
exercise and education sessions.
· Monitor and track patient progress and adjust programmes accordingly.
· Provide education on safe exercise and the benefits of regular exercise for cardiac health.
· Provide a comprehensive, exercise plan and onward referrals on discharge.
· Maintain the equipment and a clean and safe environment.
· Provide telephone support for individuals doing home based exercise programmes.
· Liaise regularly with the clinical specialist team regarding treatment progression and
ongoing care of patients.
· To maintain patient records accurately and legibly in accordance with Trust guidelines.
· To keep up to date with guidelines and best practice within cardiac rehabilitation.

Qualifications
Essential criteria
- Degree in Sports and Exercise Science or Exercise Physiology or Related Degree
- Register of Exercise Professionals Level 2 Gym Instructor and Level 3 Advanced Qualification
- Level 4 BACPR Specialist Exercise Instructor Qualification (or Equivalent)
Desirable criteria
- Registered Exercise Physiologist / Practitioner, i.e. CASES, AHCS
Experience
Essential criteria
- Experience in delivering fitness/exercise programmes to specialist populations
Desirable criteria
- Experience in prescribing exercise programmes for individuals with cardiac conditions
Knowledge
Essential criteria
- Knowledge of anatomy and physiology and how it pertains to exercise prescription
- Knowledge of long-term health conditions and how to prescribe exercise to these individuals
Skills
Essential criteria
- Excellent communication and inter-personal skills
- Ability to work autonomously and as part of a team
- Ability to prioritise workload
- Ability to respond appropriately to emergency situations
- Able to maintain accurate records
- Computer literate
- Valid UK Driving licence and have access to a car to use for business purposes (unless you have a disability as defined by the Equality Act 2010) in line with service needs
- Willingness to work flexibly and at other community sites when required.
