The Norfolk and Norwich University Hospital (NNUH) is a 1,200-bedded teaching hospital with state-of-the-art facilities for modern patient care: 1100 beds in the PFI and 100 in Cromer Community Hospital.
The NNUH is part of Norfolk and Waveney University Hospitals Group which includes the James Paget Hospital, in Great Yarmouth and the Queen Elizabeth Hospital in King Lynn.
We carry out nearly one million outpatient appointments, day case procedures and inpatient admissions annually. The population we serve is predominantly the people of Norfolk and north Suffolk, although some patients are referred from further afield especially to access specialist services available at this Trust.
Our staff of c10,000 care for and support patients who are referred to us by around 100 local GP practices and from other acute hospitals. We run more than 60 specialist services as a tertiary centre, including more than a dozen specialist cancer and radiotherapy treatments, and we are one of the biggest cancer centres in the country.
We provide complex spinal surgery services, specialist Neonatal Intensive Care, neurosciences services and specialist cardiac services. Our Trust runs specialist dermatology, rheumatology, endocrinology, adult congenital heart disease services, treatment of complex liver and pancreatic diseases in adults, cystic fibrosis services and intestinal failure services. We are enhancing our urgent and emergency care specialist services for patients with significant injuries as a result of trauma and have an ambition to become a Major Trauma Centre. NNUH is currently the busiest Trauma Unit in the East of England caring for almost 850 trauma patients annually many with complex care needs.

Job overview
This is an exciting opportunity to join our Renal Home Therapies Team as a Band 6 Nurse, supporting patients living with renal failure who choose home dialysis to manage their treatment safely at home while maintaining their independence and quality of life.
The role is varied and rewarding, combining specialist clinical care with education, patient training, and ongoing support for both peritoneal dialysis and home haemodialysis.

We are looking for a motivated nurse with strong clinical judgement, excellent communication skills, and relevant renal experience, including haemodialysis and peritoneal dialysis. You should be able to balance autonomous working with effective teamwork, be organised and adaptable, and confident supporting patients in both hospital and community settings.

Main duties of the job
As a Band 6 Nurse in Renal Home Therapies, you will manage a caseload of patients receiving home dialysis, providing specialist assessment, education, and ongoing support to promote safe and independent care at home.
A central aspect of the role is teaching patients to perform both peritoneal dialysis and home haemodialysis, enabling them to build the skills and confidence needed to manage their treatment.
You will work both autonomously and collaboratively within the Renal Home Therapies Team and wider MDT to coordinate care, evaluate outcomes, and respond appropriately to changes in patients’ clinical condition.
You will act as a primary point of contact through telephone, clinic appointments, and home visits, supporting patients and carers with symptom management, resolving dialysis-related issues, and arranging urgent reviews or admissions when needed.
The role involves clinical assessment, interpretation of results, and initiating or adjusting treatment, while promoting self-management and delivering tailored health education.
You will make key clinical decisions, including assessing suitability for home dialysis, recognising and escalating concerns, adjusting care plans in response to changes in condition or treatment tolerance, and determining when patients require admission, discharge, or referral to other services.

Detailed job description and main responsibilities
Clinical
1. To manage a specified case load of patients, providing information and support, to enable patients to make informed choices about their care and understand the rationale of investigations and treatments.
2. To work closely with the Consultants, medical and nursing staff, multi-disciplinary team and Heads of Department within the Directorate.
3. To ensure that patients are seen within recommended time scales and targets where these are in place.
4. To visit inpatients as appropriate, to ensure continuity of care and to give advice/ support.
Management and Leadership
1. To work as an autonomous practitioner, responsible for managing and prioritising own workload against the needs of patients and service requirements.
2. To assist the Line Manager in the provision of clinical and managerial leadership and support of all nursing and clerical staff.
3. To deputise for senior colleagues in their absence as required.
