Northamptonshire Healthcare NHS Foundation Trust on behalf of 3Sixty Care Partnership, and its member practices, are excited to announce that they are seeking to recruit  Senior Clinical Pharmacists across networks of GP practices in North Northamptonshire and expand the existing team.

The 3Sixty Care Partnership is a unique joint venture between a federation of 27 GP practices in Kettering, Corby, Wellingborough and East Northants and Northamptonshire Healthcare NHS Foundation Trust. We aim to bring health and care professionals together to provide community based prevention programmes and joined up health and care treatment and support for our patients where they live. 

3Sixty are working together with the five newly formed Primary Care Networks (PCNs) to support them to deliver joined-up services which we believe will lead to better quality care, better patient satisfaction and better outcomes.

We are very excited to offer opportunities for Senior Clinical Pharmacists at 8a level to work within our existing team of pharmacy professionals and to be involved in the development of new models of care. These roles will work with the existing team supporting practices in Wellingborough, East Northants, Kettering and Corby.

Main duties of the job

The role involve working in a multi-disciplinary team in a patient-facing role as well as providing primary support to general practice staff with regards to prescription and medication queries. As a senior pharmacist within the team, the chosen candidates will use leadership skills to support the development of the existing team and clinical service.  Also, the successful candidates will take responsibility for areas of chronic disease management within the practice and undertake clinical medication reviews to proactively manage patients with complex polypharmacy.

Interested applicants will need to have completion of an undergraduate degree in pharmacy and registration with the General Pharmaceutical Council, as well as a minimum of 2 years’ experience as a pharmacist, working across clinical settings. Applicants will also need to demonstrate a commitment to leadership, learning and development and an ability to adapt to different working environments.

Experience

Essential criteria
  • Experience of providing clinical pharmacy services e.g. hospital, community, primary care or skills for analysing complex patient/medicines information
  • Understanding of project management and clinical audit
  • Evidence of continual professional development
  • Experience of teaching or training staff
Desirable criteria
  • Managerial experience, including line management
  • Experience of primary care including general practice
  • Completion of relevant CPPE Primary Care Pathways
  • Understanding of relevant IT systems e.g. ePACT, EMIS, SystmOne

Qualifications

Essential criteria
  • MPharm or Equivalent
  • Registered with GPhC
  • Diploma in Clinical Pharmacy or equivalent experience (e.g. CPPE pathway)
Desirable criteria
  • Independent Prescribing qualification
  • Leadership Qualifications
