NHFT is looking for an experienced clinical pharmacist to support the delivery of a clinical pharmacy service to Spinneyfields and Turn Furlong inpatient units.

Spinneyfields provides accommodation for people requiring health and social care recovery and rehabilitation, delivered through a multi-disciplinary team employed by NHFT in a recovering independence unit.  

NHFT are the lead provider for the RIBU, collaborating with West Northants Council. The service offers support for people with an need for rehabilitation, recuperation and/or assessment; subject to the screening criteria for the service being met. It is anticipated that admissions will primarily come from acute hospital discharge (Pathway 2 as described in national Discharge to Assess guidance), either following an acute stay in hospital or being stepped out from an emergency attendance to avoid admission to hospital.

This is a varied role with the successful applicant needing to work independently and within the pharmacy team. They will have their own clinical responsibilities which will contribute to the healthcare team and support the pharmacist attending the wards. Travel between sites will be necessary and you must be able to travel independently to other bases in the Trust across Northamptonshire.

Main duties of the job

As the Advanced Pharmacist Elderly Care, Rehabilitation and Frailty your duties will include:    
•    To develop and deliver on-site clinical pharmacy services to Spinneyfields and Turn Furlong and consider and potentially implement the role of pharmacist prescribing in this area
•    To undertake medicines reviews as needed to rationalise complex regimes 
•    To be responsible for leading, delivering and evaluating clinical pharmacy services to patients within Spinneyfields and Turn Furlong
•    To provide a clinical pharmacy service to Spinneyfields and Turn Furlong inpatient areas 
•    Where a prescribing qualification is held, to make changes to prescribed medication within agreed competence as required and monitor the patient’s progress.

Qualifications

Essential criteria
  • Registered with GPhC as a Pharmacist
  • MPharm degree, or equivalent pharmacy qualification
  • Post-graduate diploma in clinical pharmacy (or equivalent level of knowledge and experience)
  • Evidence of recent continuing professional development
Desirable criteria
  • Member of Royal Pharmaceutical Society
  • Independent prescriber

Experience

Essential criteria
  • Post registration experience within the NHS setting
  • Experience of working as part of a multidisciplinary team
  • Experience of planning, implementing, analysing, reporting and changing practice through audit
  • Experience of teaching, mentoring and tutoring
  • Experience of planning, implementing, analysing, reporting and changing practice through audit
  • Experience of data collection, analysis and report writing
Desirable criteria
  • Experience of prescribing within a specialty to optimise medicines for specific patient groups
  • Management qualification relevant to the NHS
  • Experience of developing and implementing medicines management policies and procedures
  • Experience of managing staff
