Would you like to join a team which specialises in a biopsychosocial approach to the management of long-term health conditions, including Pain Management, Fatigue Management, and Level Three (Adult) Weight Management service?

You will have the opportunity to develop and shape this exciting role whilst providing advanced clinical leadership through transformation and strong governance frameworks for Long Term Condition Management services within Powys Teaching Health Board.

The post holder will be a clinical expert in their field with proven experience as an advanced clinical practitioner and will fulfil the 4 pillars of advanced clinical practice:
