Job overview
The Falls Assessment and Prevention service within Powys Teaching Health Board has been redesigned in an innovative and exciting way, and an opportunity has arisen for an experienced Therapy Assistant Practitioner to work in the specialist area of falls prevention and intervention.
We are looking for experienced and dedicated individuals with a strong interest in falls prevention and rehabilitation of our frail older population, including those living with frailty
Main duties of the job
This role focuses on community engagement, delivering interventions, and providing education to care home staff and residents, alongside completing multi-factorial falls assessments and interventions.
The post holder will support the Falls and Frailty teams in delivering high-quality, person-centred care for individuals living with frailty. Key responsibilities include supporting frailty management across community settings, delivering falls prevention and frailty-focused rehabilitation, and contributing to personalised care plans that promote strength, balance, mobility, and independence. Strong emphasis is placed on early intervention and prevention to reduce hospital admissions and enable individuals to remain safely in their preferred environment.
Working collaboratively with the Falls Clinical Specialist, Frailty AHPs, and wider multidisciplinary team (including GPs, Occupational Therapists, Physiotherapists, Nurses, Social Care, and third sector staff), the role supports a coordinated pathway for frailty and falls management.
Applicants should have relevant community experience, strong communication, and the ability to work independently and within a team. Flexibility, excellent organisational skills, and participation in a 7-day rota are essential. A driving licence and access to a vehicle are required

Qualifications
Essential criteria
- Level 3 NVQ / A-levels / BTEC in a health care associated certificate/ Diploma in Occupational Therapy Support / Physiotherapy Rehabilitation
- Good understanding of theoretical concepts of Occupational Therapy and Physiotherapy
Desirable criteria
- Evidence of continuing personal development relevant to the role in health care
- Local knowledge of statutory and voluntary services
Experience
Essential criteria
- Significant experience and skills equivalent to level 4
- Experience in providing care and undertaking clinical observations
- Evidence of working as part of a team
