Job overview

We have an exciting opportunity for a highly motivated and suitably experienced individual to join the high performing Stroke and Neurology therapy team at DBTH. 

The post holder will be involved in the assessment, treatment, and case management of Neurological patients in the hospital, located across the intensive care, medical/surgical and acute wards - on both Doncaster and Bassetlaw sites.

The successful candidate will have a proactive 'can-do' attitude and be motivated to provide high quality rehabilitation in addition to facilitating quality, effective, and safe discharges. Excellent communication skills are essential along with ability to motivate and empower both patients and other members of the team.

The successful applicant will work with flexibility and autonomy in delivering  rehabilitation programmes in addition to completing onward referrals and liaising with specialist units playing an integral role in patient flow and discharge planning .

The Neurology Therapy team at DBTH  works side-by-side, in true partnership, with Social Care, Neurorehabilitation services and community services, identifying the most appropriate pathways for ongoing patient care.

Main duties of the job

Supporting the delivery of high quality rehabilitation to the Neurology Therapy caseload across DBTH. Working,  both independently and in conjunction with registered clinicians.

To facilitate patient flow, discharge planning and the delivery of specialist therapy intervention for Neurorehabilitation patients at DRI and BDGH

To communicate and work collaboratively with relevant teams at DBTH in addition to external agencies / services to facilitate timely transfer of care. 

With oversight from registered clinicians , 

  • manage own caseload and to provide a high standard of assessment, management, treatment and advice to meet the needs of patients within agreed guidelines and protocols.
  • Case manage complex  patients on Neurology therapy caseload across DBTH.

Experience

Essential criteria
  • Active involvement in discharge planning
  • Recent experience of working as part of a multi-disciplinary team, with an understanding of the roles and responsibilities of the members of the team
  • Experience of carrying out a range of assessments independently
  • Understanding and engagement with patient flow
  • Experience working in a rehabilitation environment within an NHS setting
Desirable criteria
  • Involved in Service development
  • Experience of working in Stroke / Neurorehabilitation
  • Broad experience of working as a Band 3 / 4 Therapy assistant

Qulaifications / training

Essential criteria
  • NVQ level 3 in Diagnostic and Therapeutic Support (or Care with relevant Physiotherapy Units)/Allied Health Professionals (Occupational Therapy and Physiotherapy) or equivalent health and/or social care qualification or relevant experience
  • Portfolio evidence of relevant experience/ Continuing Professional Development in specialist field
  • IT skills
