CDC Pathway Administrator

Job overview
An exciting opportunity has arisen for a full-time Pathway Administrator to support the management team at the Community Diagnostic Centre (CDC) at Montagu Hospital.
This vital role involves working closely with a wide range of clinical and non-clinical colleagues to ensure accurate and efficient management of patient information and pathway tracking systems. A key responsibility will include the coordination and validation of data transfers between local Trusts to support seamless patient care and follow-up.
The successful candidate will play a central role in the implementation and administration of pathway tracking for the CDC, ensuring that all patients are reviewed and managed in a timely manner in line with national standards and local targets.
We are seeking a highly motivated individual who thrives in a dynamic, fast-paced environment. You will need to remain calm under pressure, meet tight deadlines, and possess excellent organisational and communication skills.
Main duties of the job
- Ensure robust monitoring of patients on active ICB pathways
- Attend meetings to provide updates on ICB pathways relevant to role
- Monitor and track onward referrals to ensure timely receipt internally and externally
- Maintain electronically stored patient information on spreadsheet or database, for audit and patient management purposes. Produce reports/information as required.
- Ensure all manual and electronic patient records are maintained accurately, including input of data to information systems used within the trust, Hospital PAS, Endosoft, Meditech and ICE.
- To provide dedicated administrative services to support ICB pathways.
- Be aware of, and work with the relevant operational leads to ensure effective delivery of relevant communication/performance targets.

Knowledge & Experience
Essential criteria
- Previous experience working in a clerical/administrative role
- Knowledge of medical terminology
- Demonstrable working knowledge of Microsoft packages including Word
- Customer care experience
- Ability to work to and promote Standard operating procedures / performance targets
- Experience of working within a busy/demanding environment. E.g. Multi-disciplinary team.
Desirable criteria
- Thorough knowledge of health care system and processes
- Understanding of Primary Care, GP and Community Services
- Knowledge of Referral to Treatment Patient Access Policies and procedures
- Understanding of patient priorities and targets in relation to appointments and waiting times, specifically Referral to Treatment
Qualifications & Training
Essential criteria
- Minimum of 3 GCSEs including English and Maths at Grade C or above or suitable equivalent qualification/relevant experience
- RSA III or NVQ III or suitable equivalent level administrative qualification/relevant experience
- ECDL or equivalent qualification / demonstrable experience
Desirable criteria
- Business Administration Level II Apprenticeship
