This is an exciting opportunity within DBTH to join the high performing MDT at the Fred and Ann Green Rehabilitation Centre based at  Montagu Hospital , part of Doncaster and Bassetlaw Teaching Hospitals NHS Trust .

The Fred and Ann Green Rehabilitation centre is a purpose built  unit on the journey to achieving  centre of excellence status. Facilities within the centre include 2 inpatient wards in addition to a dedicated outpatient department,  assessment rooms and a state of the art rehabilitation  gym. Arranging a guided visit to the centre is recommended and would be facilitated as a priority.

The  team based at the Fred and Ann Green rehabilitation centre prides itself on delivering high quality and evidenced based  interventions to maximise patient potential. The successful applicant should be passionate about rehabilitation and working as part of an MDT committed to personalised, goal focused and patient-led care.

The Rehabilitation centre has close links with Sheffield Hallam University and is keen to be involved in  research and development opportunities. The successful post holder should be willing to play an active role in this exciting transformation.

Prif ddyletswyddau'r swydd

The post holder will provide specialist assessment and individualised rehabilitation of patients with a wide variety of clinical conditions. Core skills in general rehabilitation are essential including relevant orthopaedic and frailty experience. The post holder should have a flexible, positive and pro-active approach and be able to evidence  excellent organisational, interpersonal and team working skills. Complex patient management and timely discharge planning  are also essential skills that the post holder should possess.

The post holder will contribute to the current 6/7 day service at the Rehabilitation centre.

The post holder will work within a high performing  integrated multidisciplinary team to  provide  high quality, goal-focused rehabilitation . Applicants must demonstrate a strong commitment to evidence based practice, innovation and have a patient-centred approach.

There will be opportunities to participate in quality improvement and innovation projects. At the rehabilitation unit we have a  robust CPD and Appraisal processes in place. You will be supported to develop your clinical, non-clinical and leadership skills.

The post holder will contribute to the education, training and development of Band 5 Therapists, Therapy Assistants, Multidisciplinary staff and students. The role will include undertaking professional development appraisals and service evaluation, including audit.
