Job overview

To manage and drive the delivery of a patient-centred personalised care and risk stratified pathway programme for identified cancer patient groups.

The postholder will support the Lead Cancer Nurse in managing and supporting the roll out of the remote surveillance pathway for defined patient groups within Surrey and Sussex NHS Trust (SaSH). The post holder will work across professional, departmental and Trust boundaries as required to develop and support patient-centred, seamless care, with particular focus on the delivery of Personalised Care, patient self-management, completion of holistic needs assessments, implementation of end of treatment summaries and the organisation of health and wellbeing provision.

The postholder will oversee the strategic development of the Macmillan Centre, ensuring it provides a welcoming, effective and supportive environment for patients, carers, and staff. They will work to strengthen the visibility, use, and integration of the Centre within cancer services and the wider community.

A key aspect of this role will be to manage the development of Personalised Stratified Follow-Up (PSFU) pathways, providing assurance that the patients on remote surveillance pathways are monitored safely. They will develop, with the implementation of a steering group, an implementation plan to support new teams within SASH to develop their new pathways.

Main duties of the job

The post holder will also lead on planning, promoting, and facilitating stakeholder events that enable dissemination of good practice on supported self-management programmes. E.g. health and well-being events, the recovery package and the remote surveillance pathway
