Job overview
We are delighted to offer this exciting full time post in partnership with Newcastle Outer West Primary Care Network. The post is supported and resourced by the trust but based in the Outer West Primary Care Network. The successful candidate will be joining an established team of Pharmacists and Pharmacy Technicians in a patient facing role. You can make a real difference for multi-comorbid individuals and those at risk of medication related harm. You will be supported in practice and on the Primary Care Pharmacy Education Pathway by a named senior clinical pharmacist and an educational supervisor from CPPE as well as our talented, research-active multidisciplinary teams.
Outer West Primary Care Network comprises of 6 practices, travelling between practices would be required. If you are kind, innovative, work well in a team, find patient consultations rewarding and understand patient centred care then you could be our perfect fit.

Main duties of the job
The successful applicants will register with Centre for Postgraduate Pharmacy Education’s Primary Care Pharmacy Education Pathway (PCPEP); the course is fully funded by Health Education England, and we will provide full financial support for travel, study leave, and any equipment needed.
Key responsibilities of the role include solving medicines related problems for patients and alleviating GP workload. This will include:
- reconciling medicines at the point of registration to the practice and liaison with the team regarding onward monitoring.
- Establishing patient safety systems within the practices related to high risk medicines
- making changes to the patient’s primary care record
- issuing prescriptions
- talking to patients about new/changed medicines
- electronic transfer of care to community pharmacy
- providing alternative medicines during stock supply shortages
- adjusting medicine formulations for swallowing difficulties or MDS provision
- bringing medicine issues in line for repeat dispensing or proxy ordering
- re-authorising repeat medicines
- Setting up electronic repeat dispensing
- Supporting the wider pharmacy team through collaboration and shared learning

Detailed job description and main responsibilities
- To practice as an experienced pharmacy technician.
- To practice as a registered pharmacy technician.
- To provide a clinical service to identified patients
- In domiciliary general practice or Trust settings including reducing risk associated with the admission and discharge process.
- To undertake a broad range of technical pharmaceutical activities including providing advice to patients and other healthcare staff.
Please note it is a requirement of The Newcastle upon Tyne Hospitals NHS Foundation Trust that all successful candidates who require a DBS for the post they have been offered pay for their DBS certificate. The method of payment is a salary deduction from your first month’s pay.

KNOWLEDGE
Essential criteria
- Knowledge and understanding of all technical aspects of pharmacy patient services, supported by practical experience gained by practical experience of an approved training programme.
- In addition successful completion of an in-house competency based training programme or previous relevant post qualification relevant clinical experience which provides an equivalent level of additional experience.
SKILLS
Essential criteria
- Excellent oral and written communication
- Excellent customer care
- Working knowledge of pharmacy, hospital and/or general practice computer systems.
EXPERIENCE
Essential criteria
- Practical experience of working in a relevant clinical pharmacy environment.
Desirable criteria
- Practical experience of working as a designated checker
- Familiar with Medicines Information enquiry answering resources
QUALIFICATIONS
Essential criteria
- S/NVQ Level 3 in Pharmacy Services and approved underpinning knowledge e.g. BTEC in Pharmaceutical Science or equivalent recognised pharmacy technician qualification plus experience to degree equivalent.
- Registered with GPhC
Desirable criteria
- Medicines Management qualification
- Formal management qualification
- NVQ Assessor qualification
- An Accredited Accuracy Checking Qualification
