Job overview
We are seeking a dynamic, highly skilled Specialist Pharmacist for Medicine and Anticoagulation to join our progressive pharmacy team.
This is a unique opportunity for an exceptional and highly motivated pharmacist to work with the clinical pharmacy team to shape and develop clinical pharmacy services
As the Specialist Pharmacist for Medicine and Anticoagulation, you will lead, manage, and develop the clinical and patient focused Pharmacy Services to cardiorespiratory, elderly care, stroke and rehab, neurology, and anticoagulant services, ensuring patients receive safe, high quality, and clinically effective pharmaceutical care.
You will be instrumental in advancing clinical pharmacy services and improving quality of patient care, promoting evidence-based and cost-effective prescribing, and ensuring the patient’s experience remains at the heart of our services.
The pharmacy service has a high profile within the Trust and is well integrated into multi-disciplinary teams, providing a much valued contribution to patient care, with a track record of achieving quality, operational and financial targets. Main duties of the job
1. To provide a specialist clinical pharmacy service to designated medical wards, clinics and specialist areas in conjunction with other clinical pharmacists
2. To lead the safe, rational and cost-effective prescribing of medication in medicine
3. To support clinicians with the management of patients in a specialist area, including assessing of patient therapeutic response, blood result interpretation, and adjusting medication and doses in accordance with local procedures and in liaison with clinical leads.
4. To be responsible for the Anticoagulation Service
5. To work collaboratively with pharmacy and clinical teams both within the hospital and across community settings.
6. To deputise for the Lead Pharmacist, Clinical Services, training and education, rotas and day to day management of pharmacy Clinical Services.
The post holder will be required to participate in the department’s on-call and weekend rota.

Qualifications
Essential criteria
- Registered as a Pharmacist with GPhC
- Pharmacy Degree
- Post-graduate Diploma from an approved clinical pharmacy scheme, or equivalent experience
- Mandatory CPD to maintain fitness to practice
- Independent Prescriber
Desirable criteria
- Membership of UKCPA
- Membership of the Royal pharmaceutical Society
- Postgraduate Masters
Experience
Essential criteria
- Experience as a practicing clinical pharmacist within an acute hospital trust
- Analysis and management of drug expenditure
- Experience in delivering risk reduction initiatives
- Experience of anticoagulation
