Job overview

We have an amazing  opportunity   to  work  differently to  support  our Craven Residents.
The Team Leader for Enablement and IMC will work across 
community settings, predominantly within the Craven Area, 
but also supporting the wider Community Rehab Team as 
required . 
Providing support and leadership, along with existing team 
leaders, to lead and manage an integrated team of 
occupational therapists, physiotherapists, and support staff. 
To enable Home First discharges from Hospital Settings 
and Provide Rehab services in our Craven Intermediate care 
beds. The community rehab team provides specialist 
assessment, treatment, and rehabilitation in intermediate 
care beds and in patients own homes. Blending boundaries 
and developing intermediate care roles with the Local 
authority 
This role is key in facilitating safe and timely hospital 
discharges local hospitals. In conjunction with Craven 
Enablement services as well as ensuring IMC beds are used 
to best effect and are supported to ensure best use of 
resource
Applicants will need excellent communication skills, a good 
understanding of the health care agenda and a vision for 
service development and growth.

Main duties of the job

To undertake the role of the Team Leader; managing, 
planning, coordinating, delivering and evaluating therapy 
plans as part of this broad MDT. Deputising for the 
Service manager as required


Provide support and leadership, along with existing team 
leaders, to lead and manage an integrated team of 
occupational therapists, physiotherapists, and support staff


To ensure the team provides high quality assessment, 
treatment and rehabilitation to people in their own homes 
and IMC beds 

To act as a source of expertise and provide an advisory 
service to other ANHSFT therapists and members of the 
MDT in the management of Home First and IMC patients.

To lead in the advanced assessment and treatment of 
patients within the speciality who may have complex and 
chronic presentations, and determine clinical diagnosis 
and any therapy treatment indicated, and to maintain 
records as an autonomous practitioner.

To provide expert training and education for all grades of 
staff and other health professionals in the therapeutic 
management of community rehab patients.

Provide effective student training and development.

Provide a key role in partnership working with patients, 
carers, the MDT, bed managers and wider statutory, 
private and voluntary sector services

Qualifications

Essential criteria
  • Degree or equivalent pre-registration award in Occupational Therapy
  • State registrations with the Health Care Professions Council
  • Recent post graduate courses relevant to the management of Stroke patients / Vocational rehabilitation.
  • Recent post graduate courses in Leadership/management
  • Completion of course in Professional Practice education
Desirable criteria
  • MSc or PhD in a relevant subject
  • Certificate in Managing Health Services or equivalent
  • Recent post graduate courses or experience relevant to rehab/ reablement / IMC

Experience

Essential criteria
  • Comprehensive, relevant and recent experience working in a wide variety of clinical areas
  • Experience in assessment, treatment and management of people in rehab/ reablemnet/ IMC settings
  • Experience of multidisciplinary and interagency team working
  • Experience of multidisciplinary / interagency guideline development, implementation and audit
  • Experience of clinical supervision both as supervisee and supervisor
  • Experience of supervising other staff members and conducting annual reviews
  • Recent experience of student training
  • Recent experience of leadership /management role
  • Experience of taking part in or leading on Projects/ service improvements
Desirable criteria
  • Experience of taking part in or leading an audit or research project
  • Experience in dealing with Human Resource issues such as recruitment and retention, performance management, sickness and annual leave
  • Experience of working in partnership approach working in collaboration across system barriers
