Job overview
This is an exciting opportunity for an enthusiastic and motivated Physiotherapist with relevant experience to join our established integrated intermediate care team providing an Urgent Community Response and Virtual Ward pathway. The team also provides therapy support to patients at Castleberg Hospital.
The service receives referrals from a wide range of health and social care professionals, with the aim of supporting patients with a health-related crisis at home. The team provide an urgent community response service to avoid unnecessary hospital admissions and a virtual ward to support patients to remain at home wherever possible and facilitate safe hospital discharge.
We also provide support to patients who require an out of hours district nursing service and provide input to people requiring intermediate care beds in the community.
Main duties of the job
Provide a personalised intermediate care service to adults in their own home within Airedale, Wharfedale and Craven areas. This also includes providing therapy support to patients at Castleberg Hospital.
Perform physiotherapy assessment of patients with a diverse range of presentations and complicated physical and psychological conditions, to provide a diagnosis, develop and deliver an individualised treatment programme in conjunction with the rest of the team.
Provide a key role in working in partnership with medical colleagues, multi-disciplinary teams (MDT) and carers to set realistic objectives and to help to facilitate goal and discharge planning to enable safe discharge from our service.
Work to support the team in its delivery of interventions, to skill share with other professionals to support the full breadth of the service (competency training is offered to support this skill sharing method of delivering intervention).

Qualifications
Essential criteria
- Degree or equivalent pre-registration award in Physiotherapy.
- UK Registrations with the Health Care Professional Council.
Desirable criteria
- Relevant post-graduate courses
- Member of the Chartered Society of Physiotherapy
- Member of Physiotherapy special interest groups/organisations
Experience
Essential criteria
- Recent and relevant undergraduate experience working in a variety of clinical areas and settings.
- Undergraduate experience of multi-disciplinary team working and working collaboratively with other professions and agencies.
Desirable criteria
- Experience in use of IT applications, eg SystmOne.
- Recent post graduate experience with relevance to the job.
- Community or in-patient rehabilitation experience.

Knowledge

- Knowledge of standardised and non-standardised assessments and treatments relevant to Physiotherapy
