This may include supporting discharge from hospital, responding to periods of crisis, or providing reablement interventions to help people regain independence.
A key aspect of the role is ensuring people receive the right support at the right time, with regular reviews to ensure changing needs continue to be met. You will also support and supervise junior physiotherapy staff, helping to develop their clinical skills and confidence.

- Working as an autonomous practitioner while contributing effectively within the multidisciplinary team.
- Supervising, supporting and developing staff, support workers and physiotherapy students.
- Contributing to service development, audit, quality improvement and clinical governance activities.
- Maintaining accurate clinical records and ensuring practice is aligned with professional and organisational standards.
