Job overview

Working with the multidisciplinary Pulmonary Outreach Team that includes a physiotherapist, Respiratory Nurse Specialists and a Clinical Support Worker in Wrexham Maelor Hospital of BCUHB.

To assess patients’ suitability for the service, managing exacerbations in the home settings and referring to the other professionals as appropriate.

To undertake a comprehensive specialised respiratory assessment of patients using investigative and analytical skills and to formulate individualized physiotherapy management and treatment plans for patients, where the caseload will comprise of mainly of COPD patients with complex presentations. This will be through specialist clinical knowledge and application; audit, research and education. It will require liaison with a diverse number of multidisciplinary specialties and integrated working with other disciplines and agencies.

 

Main duties of the job

To work as an autonomous, respiratory physiotherapist. To work with the Lead Nurse Respiratory Services and team to develop and deliver a high-quality clinical service throughout BCUHB. Leading by example; provide a high standard of specialist assessment and treatment of patients in a variety of locations. Using evidence-based principles/ current best practice and advanced clinical skills and clinical reasoning to assess, plan, implement and evaluate interventions.

 

Clinically reason and effectively treat routine cases and identify those with complex needs where support may be required.

 

Deliver some elements of Occupational Therapy and Nursing elements, within previously agreed protocols. For example, assessing, ordering and issuing specific equipment and performing venepuncture for various investigations.

 

To participate in clinical education and supervision for physiotherapy students, other healthcare/research students in the department, and acting as a role model and clinical resource demonstrating high standards of care and providing clinical leadership in the specialist area.    

To advise on the management, investigation or onward referral of patients referred to the Pulmonary Outreach Team.

Support the Lead Nurse Respiratory Services and to plan new projects   to develop, co-ordinate, audit and evaluate the PORT service to continuously improve clinical practice, bring about change and provide the necessary assurance and governance within the Pulmonary Outreach Team.

Keeping up to date with recent advances in the management of COPD and physiotherapy practice and to promote the use of evidence-based practise.

Qualifications and/or Knowledge

Essential criteria
  • Diploma/ Degree in physiotherapy, HCPC registration, Advanced accredited training at masters level or equivalent, Accredited post graduate specialist courses/ training relevant to role.

Experience

Essential criteria
  • Significant post-graduate experience relevant to specialty, Significant experience managing a complex caseload working within relevant specialty, Experience in multi-agency team working within specialist area, Evidence of supporting clinical governance requirements including codes of conduct

Aptitude and Abilities
