Job overview

An exciting opportunity to join our Cancer Services Team and play an integral part in our patients' journey.

To input referrals onto the Trust Cancer database ‘SCR, virtually track patients through the first out-patient appointment and diagnostic test to ensure no delays occur and collect case notes for week Multi-Disciplinary Meetings

Main duties of the job

Input Cancer Fast Track Referrals onto the Trust Cancer database, SCR (Somerset Cancer Register)

 Check patients attend for their first out-patient appointment and / or diagnostic tests.  Where patients fail to attend, request a new appointment to be given.

 Retrieve clinical information from various sources, e.g. computerised hospital systems, case notes, patient documentation, and other hospitals/hospices personnel on a daily basis.

Ensure that the results of investigative procedures, appointment dates, operative procedures etc are input onto the databases in a timely and accurate manner to a given deadline.

 

1.5          Ensure that all data entered and stored within the database and within the department is a precise and accurate reflection of the patient’s pathway through the hospital ensuring that an audit trail of the data collected is available.

 Liaise with Cancer Consultants, Cancer Nurse Specialists, Clinicians, Nurses, Secretaries, and other appropriate personnel from within and outside the Trust to ensure complete data capture.

 Collect case notes for the weekly MDT’S and prepare for the MDT team so that patients can be discussed in a timely and efficient manner.

 To ensure that Local and National cancer waiting time targets are achieved by highlighting possible delays in the individual cancer patients journey and notifying the MDT Co-ordinator of such, so necessary action can be taken to prevent delays in the patients’ journey.
