Main duties of the job
We are looking for an experienced community nurse who has a real enthusiasm for caring for patients in their own homes in the band seven team leader role.
You will be required to lead a team of Community Nurses and manage the day to day, on-going delivery and development of the team.
The applicant will support the leadership of the service within the structure and will provide a service that is responsive, high quality, supporting practice delivery and continually improving standards within the Clinical Governance agenda.
You will work with clinical and professional leads to implement best practice and ensure the maximum utilisation of resources to benefit our patients. You will be a role model for junior staff, you will be responsible for moving the team into new models of care working with colleagues within your neighbourhood.

Detailed job description and main responsibilities
JOB SUMMARY:
The post holder will be responsible for managing the deployment and supervision of an integrated
team within a defined clinical area/geographical community.
Responsibility for the leadership and management of the day-to-day delivery of services including
managing issues relating to HR, performance management, compliance with Care Quality
Commission Standards and delivery of service key performance indicators.
Working under the direction of the Community Therapy and Nursing Matrons and with colleagues
and key stakeholders to ensure the delivery of a coordinated, multidisciplinary and multi-agency
service. This includes planning, delivering, evaluation and development of the services for which
they are responsible.
Monitoring and analysing performance data against agreed targets for the team.
Developing and agreeing action plans to address performance issues.
Managing a defined clinical caseload if appropriate.

Essential criteria
- Clinical qualification to degree level or equivalent
- Training and experience in clinical management
- Evidence of ongoing professional development
- State registration with the Nursing and Midwifery Council
- 2 years experience at Band 6 in the community
