Job overview
This post provides an excellent opportunity for an enthusiastic, dynamic and motivated pharmacist with a clinical pharmacy background to join the
Post-Acute Medicine team.
The post holder will work as part of a dedicated pharmacy team to provide and develop specialist clinical pharmacy services within post-acute medicine. The post holder’s specific focus will be on developing and delivering care at home for patients for patients with cystic fibrosis or respiratory disease, and more broadly as future needs arise. The post holder will liaise between wards, departments and pharmacy to ensure that pharmacy services are tailored to meet the needs of of the Care Group wherever possible.
Main duties of the job
The post holder will participate in the following services:
- Act as a deputy for Principal Pharmacist when necessary.
- Participate in the multidisciplinary team delivering and promoting high quality, cost-effective, patient-focused pharmaceutical care to patients, including those receiving homecare.
- Provide interface and discharge liaison, and support to the wards & units within the Care Group.
- Consistently lead on the provision of clinical pharmacy services to the many sub-specialties within the Care Group and ensure a high level of clinical pharmacy input.
- Act as a clinical role model, and supervise rotational staff at ward level.
- Promote the safe drug use within the specialty.
- Develop, implement and audit local guidelines.
- Audit usage of medicines in the Care Group with the aim of promoting cost effective prescribing
- Assist in the financial reporting to the Care Group and drug budget management.
- Ensure seamless pharmaceutical care on discharge, liaising with other pharmacists and healthcare professionals (primary and secondary) taking over care of the patient.
- Work with the Principal Pharmacist Post Acute Medicine to extend the boundaries of professional practice within the Care Group.

Education and Qualifications
Essential criteria
- Registered Pharmacist (Significant competency assessed and examined professional registration with the General Pharmaceutical Council)
- Postgraduate Certificate in Pharmacy Practice or equivalent qualification/experience
- Working towards a Postgraduate Diploma in Pharmacy Practice or Clinical Pharmacy
Desirable criteria
- Postgraduate Diploma in Pharmacy Practice or equivalent qualification / experience
- Registered Independent Prescriber or willingness to complete
- Member of the RPS faculty
Knowledge and Experience
Essential criteria
- Specialist clinical knowledge and skills relevant to area of practice
- Appropriate post registration experience in hospital Pharmacy or equivalent area of practice with assessed competencies
- Experience of supervising others
- Experience in directorate pharmacy
- Experience of practice research or audit
- Experience of protocol/guideline development
- Experience of delivering training and education at postgraduate level
- Experience of multi-disciplinary team working
Desirable criteria
- Previous relevant specialist clinical experience
- Demonstration of advanced practice using evidence mapped to the RPS Advanced Practice Framework
Skills and Competancies
Essential criteria
- Be able to impart knowledge on medicines use to other healthcare professionals
- Be able to influence senior medical staff, the multidisciplinary team and management
