Job overview

The post holder will work autonomously and in collaboration with other health care professionals, statutory and voluntary agencies, and the planned care locality teams to deliver excellent standards of patient care for patients requiring support in unplanned care. 

  •  To Deliver high quality, skilled care to patients at home. 
  •  Work shift patterns including weekends and bank holidays. The service operates through pre-rostered shift patterns spanning 8AM – 10PM 7 days a week.
  • To utilise specialist skills and knowledge to holistically assess needs and provide short term programmes of nursing care for vulnerable adult’s base on evidenced based practice.
  • To Work as required on a flexible basis to cover all roles required within the team including. 
    -    Frailty Hospital at Home (H@H)
    -    Early intervention vehicle (EIV)

Main duties of the job

  • To Work within and across professional and organisational boundaries, liaising with GP’s, other professionals, and voluntary agencies to deliver individualised and expert clinical care to a high standard.
  • To support the locality manager/clinical lead and colleagues in delivering high quality care that delivers best value and meets the requirement of commissioners.
  • To take responsibility for managing members of the team, including undertaking appraisals with the support of senior staff.

Education/Qualification

Essential criteria
  • Registered Nurse with current NMC registration OR Paramedic with HCPC registration.
  • Evidence of recent and continuing post-registration professional education and training.
Desirable criteria
  • Experience of working in Primary Care/Accident & Emergency
  • Experience of assessing and providing quality care to people with long term condition.
  • Qualifications in Community and/or Accident and Emergency health care.
  • Qualification in teaching or practice based learning.
  • Physical Clinical Assessment Skill Course or willingness to undertake the course.
  • Independent Prescribing qualification.

Experience

Essential criteria
  • Evidence of recent substantial post-registration professional clinical experience
  • Extensive range of clinical skills and experience including, e.g. IV therapy, venepuncture, cannulation, rehabilitation, or willing to undertake the courses within CLCH.
  • Experience of working within a multi-disciplinary team
  • Experience of working with people with complex health needs
  • Experience of supervising junior staff and co-ordinating daily activities on a regular basis
  • Experience with working autonomously and independently in the community.
  • Experience of undertaking comprehensive health needs assessment and writing care plans
Desirable criteria
  • Experience of implementing evidence-based care in practice

Skills and Knowledge

Essential criteria
  • Ability to communicate effectively both verbally and in writing
  • Has high level of competence in a range of clinical skills
  • Able to work without direct supervision, planning and prioritising own workload and that of others.
  • Ability to identify own and others learning and development needs and actively seek ways to meet those needs
  • Principles of care for patients with long term conditions, relevant NSF’s and other national standards
  • NMC/HCPC Codes of Professional Conduct and application to practice
  • Managing difficult situations and conflict resolutions
  • Hold a full, valid, UK driving licence and have access to a car to use for business purposes (unless you have a disability as defined by the Equality Act 2010)
