Job overview

The post holder for the  lead Nurse  of the Resilience, Acute Flow and Transformation directorate sitting within the Corporate Division, working effectively with fellow ward teams and Divisional Matrons to deliver quality services for patients, efficiently. There is a specific focus on the deteriorating patient and improving outcomes for patients.

The post holder will   lead the Acute Response Team,  providing an exemplary service to patients.

The post holder will be responsible for the assessment, planning, implementation and evaluation of care to acutely deteriorating and critically ill patients. This includes non-medical prescribing, ultrasound guided cannulation, arterial blood gas sampling, spirometry out of hours, review of all tracheostomies/laryngectomy patients, liaising with Critical care.

Main duties of the job

  To provide clinical and managerial leadership for nursing and other healthcare professionals.

·                       Line manage Acute Response Practitioners and or other support roles within the Acute response team

 

·       The post holder will provide advice and support to wider members of the multi-disciplinary team.

·       To perform delegated tasks on behalf of the matron in their absence

·       To support with education for Martha’s rule in the Acute Response Team and with wider organisation

·       To ensure risk assessments are completed alongside the matron

·       To facilitate the stepdown of patients from Critical Care to ward areas.

·       To provide informal and formal education to all healthcare professionals within the Trust.

·       To proactively manage the clinical workload at night as necessary.

·       The post holder will be required to rotate between both sites that make up Calderdale & Huddersfield NHS Foundation Trust.

Detailed job description and main responsibilities

·       The post holder is responsible for leading the  acute response team to ensure safe and effective patient care.

·       Management  of  team

·       To assess, plan, implement and evaluate care to deteriorating patients referred by the ward teams and NEWS2 escalation.

·       Work closely with the medical, surgical and critical care registrars and junior teams, attending medical handovers.

·       Provide expert clinical advice and guidance to ward staff in the support of patients 24/7.

·       To provide informal and formal education to all staff regarding recognition and care of deteriorating patients and acutely ill patients.

·       Manage and support the Nursing associates triage request via Task Management system to coordinate the delivery of care in Out of Hours period.

·       Establish and maintain effective communication with all members of the multi-disciplinary teams.

·       To liaise and work closely with medical and nursing staff within Critical Care.

·       Work in partnership with ward managers to ensure that prescribed standards of patient care are met.

·       To support the Clinical lead with Quality Improvement projects across the trust, taking the lead when required.

To provide relevant and clear information, education and advice for patients, carer’s and family at all times

QUALIFICATIONS AND TRAINING

Essential criteria
  • Independent Prescribing or to complete this in within 2 yrs
  • First level registration & current NMC registration
  • Evidence of post registration study and development in acute or critical care
  • Teaching and assessing post registration qualification
  • PG- Certification in acute care/Critical care course or willingness to do this within 2 years

KNOWLEDGE, EXPERIENCE AND EXPERTISE

Essential criteria
  • Extensive & recent experience in acute/critical care setting
  • Advanced clinical knowledge
  • ALS certificate (or willingness to immediately undertake)
  • IRMER (Ionising Radiation), willingness to undertake to enable x-ray requesting
  • Evidence of leadership skills
  • Competent in cannulation & phlebotomy or willingness to undertake in 3 months including ultrasound guided cannulation
  • Competent in male/female catheterisation or willingness to undertake in 3 months
  • Competent in arterial blood gas sampling or willingness to undertake in 3 months
  • CVAD trained competent
  • Verification of Death or willingness to undertake in 3 months
Desirable criteria
  • Competent in spirometry

COMMUNICATION AND RELATIONSHIPS (INCLUDING MANAGEMENT RESPONSIBILITIES)

Essential criteria
  • Excellent written and verbal communication skills.
  • Understands complex clinical and technical problems and exercises specialist clinical judgement when assessing and interpreting specialist and acute patient conditions
  • Competent in use of Nerve centre
  • Has self -confidence and uses this to lead, involve and challenge others as appropriate
  • Displays a strong motivation and commitment to success
