Therapy Assistant Practitioner

Job overview
We are looking for a motivated and driven therapy assistant practitioner with an interest in rehabilitation. The successful candidate will support our occupational therapy and physiotherapy colleagues to deliver a high standard of care and rehabilitation across our intermediate care beds in Dudley. You will hold your own caseload following completion of competencies and will play a pivotal role in rehabilitation, recovery and discharge planning for our patients.
The post holder will be responsible for treating patients under guidance with a wide range of clinical needs and conditions, including orthopaedic, neurological, medical and surgical, as well as frail elderly co-morbidities.

Main duties of the job
To provide effective and efficient rehabilitation under the supervision of qualified therapists to a caseload of adult patients with a variety of conditions who require community rehabilitation.
To manage active clinical caseloads or allocated cases.
To carry out non-complex assessments within scope of practice as required meet the needs of the patient.
To liaise with patients, family and carers, the interdisciplinary team, acute and community Therapies, colleagues in social services, the acute trust, voluntary sector and other agencies.
To assist with training for Occupational Therapy, Physiotherapy staff and students with the specified area.
To assist senior staff with the general supervision of band 3 Therapy Assistants as required
To actively facilitate and make recommendations regarding the safe and timely discharge of patients from Intermediate Care for patients on their caseload/allocated patients.

Qualifications & Education
Essential criteria
- Foundation degree Health and Social Care, or equivalent level 5 qualification OR NVQ Level 3 Health + social Care with equivalent clinical experience
- Significant experience of working as a therapy support worker or equivalent
- GCSE (A-C) – Math's & English or equivalent including functional skills level 2
Knowledge and Training
Essential criteria
- Able to demonstrate substantial experience in health and care settings, including in the relevant field of work.
- IT Training / knowledge
- Commitment to undertake appropriate training
- Ability to work under own initiative and in isolation Ability to teach practical skills

Experience

- Experience of care work within an acute/community setting

- Ability to engage in the safe moving and handling practices with patients and equipment
- Able to independently travel to meet the needs of the job. This includes being able to carry large pieces of equipment and falls response equipment and being able to respond within the 2hr urgent community response time frame. Note – public transport will not support all areas covered across the borough.
