10 month fixed term contract or a 10month secondment opportunity  - Covering maternity leave

We are looking for a dynamic, proactive and flexible nurse to work on the  Priority assessment Unit and Discharge Unit and be involved in the evolution of the Frailty  Same Day Emergency Care ( FIT SDEC) at the Royal Lancaster Infirmary.

You must be highly motivated, with a desire to build and enhance a proactive team, as a developing unit the role will expand as the service dictates.

This new unit provides care to a range of groups of patients:

  • Patients from the Emergency Department awaiting specialist Intervention from Medical / Surgical / Orthopaedic / Gynae teams
  • Patients who have been medically discharged from hospital care and required their discharge facilitating
  • Frailly SDEC is a dedicated unit to provide assessment for frailty patients streamed from the emergency department  

Main duties of the job

You will be required to:

·  Liaise directly with the ED / surgical / medical teams involved

· Confident to utilising Clinical Criteria for Discharge and utilising your assessment skills to facilitate this

· Link in with wards and have the confidence to go into all areas to facilitate the moving of patients suitable for the discharge unit

· Liaise with discharge teams + UCAS and utilise the STRATA system

· Liaise with pharmacy to ensure all mediations dispensed to patients are accurate and discuss any changes with the patient, escalate appropriately.

· Be able to work as an effective team member and can work flexibly

· Demonstrate effective communication skills.

· Be able to deliver high quality patient focused care and be motivated and dedicated to improving clinical practice.

· Be committed to personal and staff development, act as mentor/assessor in the clinical environment

The role includes covering 7 day working including long days, short shifts

Anyone newly appointed to the trust will be subject to the completion of a satisfactory 6 month probationary period (this includes bank staff). This will give the new staff member an opportunity to become familiar with the role and working environment.

Working for our organisation

We operate from three main hospitals - Furness General Hospital (FGH) in Barrow, the Royal Lancaster Infirmary (RLI), and Westmorland General Hospital (WGH) in Kendal, as well as a number of community healthcare premises including Millom Hospital and GP Practice, Queen Victoria Hospital in Morecambe, and Ulverston Community Health Centre.

FGH and the RLI have a range of General Hospital services, with full Emergency Departments, Critical/Coronary Care units and various Consultant-led services.

WGH provides a range of General Hospital services, together with an Urgent Treatment Centre, that can help with a range of non-life threatening conditions such as broken bones and minor illnesses.

All three main hospitals provide a range of planned care including outpatients, diagnostics, therapies, day case and inpatient surgery. In addition, a range of local outreach services and diagnostics are provided from community facilities across Morecambe Bay.

Our community services for adults are provided in people’s homes, community centres, clinics, GP Practices, community hospitals and our main hospitals. Our aim is to work with people to help them remain independent, improve their health and manage their conditions through high quality care, advice and support.
